Perinatal and neonatal morbidity of the newborns who was born after pregnancies of solid organ transplant

Abstract (EN)

Background and Aim: This study conducted to evaluate prenatal, natal and postnatal follow up results of babies whose mothers had undergone organ transplantation and followed up during pregnancy at Başkent University Faculty of Medicine, from 1993 to 2016 in Ankara Hospital. Materials and Methods: In this study, files of 440 reproductive aged (15-49 years) female patients were examined who had undergone organ transplantation and they were followed up after transplantation. Twenty-eight pregnant women were followed after organ transplantation were included in this study. Maternal (age, etiology of transplantation, time between transplantation and gestational age, gestational age-time and type, presence of assisted reproductive techniques and drugs and doses used during pregnancy) and neonatal (gestational age, birth weight, delivery room data, baby with anthropometric measurements, SGA, AGA, LGA and features, newborn intensive care-mother postpartum follow-up) features were evaluated. Statistical analyzes were performed with SPSS software package. Continuous variables summed as mean ± SD. Categorical variables are expressed as frequencies and percentages. Ethics committee approval was obtained. Results : Total 71,4% of organ transplantations (20/28)was renal and 28.6% (8/28) was liver transplant patients. 60.7% (17/28) of transplantations were from alive donors and 39.3% (11/28) of transplantation from cadavers. The most common transplant ethiology was glomerulonephritis (21.9%). 78,6% of pregnancies resulted as live births and 90.9% was normal birth. One of the patients had acute and two of them had chronic rejection. Mean duration between transplantation and pregnancy was 84,852,5 weeks. Common treatment protocol was cyclosporine + prednisolone + azathioprine. 40,9% of babies were term, 40,9% of babies were SGA. The birth weight of %22.7 infants wasbelow 1500 grams, %59 were 366/7 weeks and younger. Of the infants 4 (%18,1) had RDS, 1 (%4,5) had NEC and 3 (%13,6) had ROP. 18.2% of the infants needed resuscitation and 50,0% of infants were given to mother after the birth. Two babies (9.1%) was breastfed only, 10 (45.5%) of them was fed formula only and 10 (45.5%) infants were fed with formula and brestfeeding both. 54.5% (12 infants) needed intensive care. Congenital heart anomalies were detected in 3 of the babies. Conclusion : The data obtained from this study is valuable for perinatal and neonatal morbidity of the newborns who was born after pregnancies of kidney and liver transplant patients in our hospital.
