Abstract (EN)

Introduction and Aim: Obesity is an important public health problem that creates a serious burden on the world economy. In the fight against obesity, no conservative method is as effective as surgical intervention. Today, the most common use of laparoscopic sleeve gastrectomy increases the importance of the complications it brings. Leakage from the staple line, bleeding and pulmonary thromboembolism/pulmonary embolism are seen as the most serious early complications. In this study, we aimed to investigate the compatibility of our complication rates in the early period after LSG surgery with the studies in the literature, as well as the patient's demographic data, the number of staples we used in the surgery, and the relationship between fibrin glue and complications. Material and Method: 335 patients who underwent LSG between January 1, 2017 and March 1, 2022 in the General Surgery Clinic were retrospectively included in our study. A detailed anamnesis was taken from the patients and preoperative preparations were made together with their physical examinations. Fibrin glue was used in 311 (92.8%) of all patients who underwent the standard LSG technique. 5 staples were used in 317 (94.6%) patients, and 6 staples were used in 18 (5.4%) patients. Postoperative complications up to the first month were recorded. The data obtained within the scope of the research were analyzed in the SPSS (Statistical Package for Social Sciences) 26.0 package program. Results: A total of 335 patients, 67 men and 268 women, aged between 18 and 72, were included in the study. After surgery, leakage was observed in 2.4%, bleeding in 0.6% and pulmonary thromboembolism in 0.6%. The number of fibrin glue and staplers used in the patients was not statistically significant in terms of leakage. Bleeding was statistically significantly higher in patients not using fibrin glue compared to patients using fibrin glue. (p=0.018). Conclusion: Our postoperative leak, bleeding and pulmonary thromboembolism complication rates were found to be compatible with literature studies. Although the effectiveness of the fibrin glue we used in preventing leakage is controversial, it has a high effect on bleeding. We think that fibrin glue can be used in patients who underwent laparoscopic sleeve gastrectomy to effectively prevent bleeding from the stapler line. Keywords: Fibrin glue, Bleeding, Leakage
