Evaluation of the efficacy of three differential norepinefrine doses in casearine applications under spinal anesthesia

In this study, it is aimed to evaluate the effectiveness of norepinephrine in three different doses in cases of the cesarean section under spinal anesthesia. 100 volunteers who are between the ages of 18-40 years, in the ASA I-II group, undergoing cesarian section at elective conditions due to 37-42 weeks of gestation are included in the study. The patients have been divided into four groups. In group I, 0.05 mcg / kg bolus norepinephrine is administered as a total volume of 2 ml intravenously (iv) to protect the baseline after spinal anesthesia, in group II, 0.075 mcg/kg bolus norepinephrine is administered as a total volume of 2 ml intravenously (iv) to protect the baseline after spinal anesthesia, in group III 0.1 mcg/kg bolus norepinephrine is administered as a total volume of 2 ml intravenously (iv) to protect the baseline after spinal anesthesia, and in group IV, which is the control group, saline (0.9% NaCl) iv is administered in the same volume (2 ml) after spinal anesthesia. In the situations of systolic blood pressure < 100 mmHg and systolic blood pressure reccurantly dropping below %20 of baseline have been intervened with 5 mg ephedrine. Heart rate dropping below 60 is considered as bradichardia and treated with 0.5 mg iv atropine. Maternal blood pressures, heart rates, ephedrine and atropine consumptions, umbilical cord blood gas values, nausea/vomiting frequencies and Apgar scores of the four groups of patients have been compared. As a result, we can say that prophilactic single dosage of bolus epinephrine is effective, and 0,1 mcg/kg norepinephrine dosage is more succesful for hypotension induced by anesthesia in elective cesarian sections.
