Abstract (EN)
Introduction and Purpose: Subarachnoid hemorrhage is defined as the presence of blood in the subarachnoid space of the brain due to traumatic or spontaneous causes, mostly arterial and rarely venous. Most subarachnoid hemorrhages are caused by trauma. Cerebral aneurysm rupture is the most common cause of spontaneous subarachnoid hemorrhage. In our study, it was aimed to contribute to the literature according to the results obtained by evaluating the data obtained by retrospectively examining the epidemiological, clinical and radiological findings of patients who came to the emergency department with spontaneous subarachnoid hemorrhage, together with the national and international literature. Materials and Method : We retrospectively evaluated a group of 590 patients who came to the emergency department of Inönü University Turgut Özal Medical Center with spontaneous subarachnoid hemorrhage between 2011 and 2020 and were diagnosed with cerebral aneurysm as a result of DSA. Patients with spontaneous SAH who came to our emergency department and were found to have cerebral aneurysm as a result of DSA, age at admission, male-female gender ratios, frequency of months and seasons when they applied to the emergency department, complaints and clinical findings at the time of admission, known concomitant chronic diseases, admission GCS values, distribution according to FISHER and WFNS classifications, prognosis, length of hospital stay, and treatment methods were examined. The localizations, sizes, shapes and presence of multiple aneurysms detected after DSA were investigated. Findings: In our retrospective study, it was determined that the most frequent admission to the emergency department with spontaneous subarachnoid hemorrhage was in the 6th decade, the female-male sex ratio was 1.5/1, the most common admission time was in the winter-spring seasons, especially in December, and the mortality rate was 39%. A.cerebri media aneurysms were the most common aneurysms found in DSAs performed in our clinic. It was observed that the most common presenting complaint in spontaneous SAH was headache, and additional findings such as nausea-vomiting, epileptic seizures, focal neurological deficits, changes in consciousness, loss of consciousness werw also recorded. It was determined that epileptic seizure and loss of consciousness affected the prognosis negatively and increased mortality significantly. While hypertension was the most common chronic disease in patients presenting with spontaneous SAH, it was found that it increased mortality. In FISHER and WFNS classifications, mortality increased as grade progressed. 5 Results: Spontaneous SAH is a severe pathology that causes serious morbidity and mortality. Regardless of the surgical or endovascular interventional method, the treatment of intracerebral aneurysm is very difficult and risky, and it should be performed by an experienced team and health centers with a multidisciplinary approach. According to the statistical results we obtained in our study, the female-male ratio, the frequency of the patients' presentation, the general mortality rate, the age-mortality relationship, the most common location of the aneurysm, the seasonal distributions of spontaneous SAH development, the relationship between FISHER and WFNS with mortality were found to be consistent with the literature.
