The effect of gender on reversal of non-depolarizing block with sugammadex

In this study, we aimed to investigate the effect of gender for reversing the non-depolarizing block with sugammadex on patients who underwent surgery with general anesthesia. Total 100 patients, between 18-65 years, 50 men and 50 women in ASA risk groups I and II were included in the study. Cases were divided into groups as male (group E) and female (group K) according to the gender. Routine hemodynamic and TOF monitoring were applied to the patients who were premedicated with 0.03 mg/kg iv of midazolam. TOF-WATCH® SX device was used for neuromuscular transmission monitoring. Anesthesia induction was performed with propofol 2mg/kg, fentanyl 1 mcg/kg and 1 mg/kg lidocaine. When eyelash reflex disappeared, calibration was performed with peripheral nerve stimulator. Control TOF value was recorded. Rocuronium 0.6 mg/kg was administered intravenously. Time to TOF value of zero from rocuronium administration was recorded. Patients were intubated. Maintenance of anesthesia was performed as % 50:50 02:N20 and % 6 desflurane. When the TOF ratio recovered to % 25, rocuronium was repeated in the dose of ¼ of the initial dose. The first time to reach TOF25 was recorded. Repeated the number and amount of rocuronium were recorded. TOF values were recorded at the end of surgery as TOFend. After the end of the surgery, when the TOF ratio recovered to % 25, sugammadex 2 mg/kg was administered. After the administration of sugammadex, duration of the TOF ratio reach to %90 from % 25 (TOF25-90) was recorded. When the TOF ratio reached to %90, desflurane was discontinued. If the patient's respiratory rate was over 8 per min, EtCO2 level was less than 50 mmHg, SpO2 value was higher than 90% and tidal volume was sufficient, patients were extubated. The time from TOF ratio % 90 to the extubation called extubation time and recorded. Head lifting and tongue protrusion tests were performed all patients every three minutes for thirty minutes to evaluate postoperative residual curarization (PORC) in the postoperative care unit (PACU). The time to patients' Aldrete score ≥ 9 from arriving PACU called recovery time and recorded. Patient followed at least 30 minutes in PACU who did not have PORC and whose Aldrete score ≥ 9, was transported to service. There were no differences between the groups according to demographic data, duration of surgery and rocuronium use. Time to reach TOF0, TOF25 and TOFend was similar in both genders. There were no differences between the groups in terms of time of (TOF25-90) after sugammadex administration, extubation time, recovery time. PORC was not observed in any patient. While reversing of non-depolarizing block, after application of sugammadex it was observed that patients wake up earlier and the patients recovered faster. It was concluded that gender was not effective for reversing non-depolarizing block with sugammadex.
