The effect of tumescent use on hemoglobin values in superomedial pediculed breast reduction surgery: A retrospectif study

Abstract (EN)
Purpose: In this study, it was aimed to contribute to the literature whether tumescent infiltration under tourniquet had a significant effect on the amount of bleeding in patients undergoing breast reduction surgery with superomedial pedicle. Materials and Methods: Data of 64 patients who underwent reduction mammoplasty with Wise patterned superomedial pedicle between 01.01.2018-01.07.2019 were analyzed retrospectively. 42 of them were operated with 100 cc tumescent infiltration into both breasts. For other 22, tumescent solution was not used. Hemoglobin (Hgb), hematocrit (Hct) and platelet (Plt) values were noted from the results of whole blood counts required for anesthesia preparation procedures before the surgery. The same values taken within the first 3 hours postoperatively were also noted. Preoperative hemoglobin levels were 12.87 in Group-1 and 13.11 in Group-2. Similarly, the hematocrit values of the patients in Group-1 were 39.13 and 39.78 in Group-2. Preoperative platelet values were reported to be 294.095.24 in Group-1 and 298.136.36 in Group-2. In the postoperative period, the mean hemoglobin values of the patients in Group-1 were 11.27, the mean hematocrit values were 34.52 and the platelet count was 268.261.90. The mean hemoglobin values of the patients in Group-2 were 10.90, the mean hematocrit values were 33.51 and the platelet values were 257.454.55. Results: When the mean hemoglobin decrease in Group-1 was compared with the decrease in hemoglobin in Group-2, p value was found to be 0.012. Similarly, hematocrit difference p value was 0.02; and 0.1 for the platelet. Conclusion: Moderate bleeding occurs during breast reduction operations. Reducing the amount of bleeding allows the patient to mobilize early, be discharged earlier, and return to social life more quickly. Tumulus is an easy to use, inexpensive, effective and safe method that significantly reduces blood loss during surgery. Key Words: breast reduction, superomedial pedicle, tumescent
