Abstract (EN)

DR. AHMET MEL H AKYOLA RESEARCH ON THE EFFECT OF LAPAROSCOPIC ISCHEMIC PRECONDITIONMANOEUVRE ON OXIDATIVE STRESS IN RABBITSSUMMARYPurpose : The purpose of this study was to compare several laparoscopic stress manoeuvres withregard to formation severity of oxidative stress.Method and materials : 36 white New Zealand grown-up female rabbits, isolated from the males,weighing 3 - 4 kg, were obtained. They were randomly assigned into four groups: the control group,the 10 mmHg pnomopertoneum applied group(Pp10), the 15 mmHg pnomoperitoneum appliedgroup(Pp15), following the application of laparoscopic ischemic precondition the 15 mmHg appliedgroup( PPp15). In this study, in order to generate insufflation, 100% CO2 gas was used at astandardized 1 lt / min. insufflation speed. The control group was applied anesthesia for only 105minutes. The Pp10 group and the Pp15 group were respectively subjected to a 90-minutepnomoperitoneum with 10 mmHg and 15 mmHg followed by a 15-minute desufflation. To thePPp15 group, first, Laparoscopic ischemic precondition manoeuvre, then, with 15 mmHg,pnomoperitoneum and a 15-minute desufflation were applied. Laparoscopic ischemic preconditionin this research was considered as a 10-minute desufflation after a 10-minute 15 mmHgpnomoperitoneum. Periatel periton biopsy was taken laparoscopically at the beginning and at the20th minute of the experiment, in all four groups, With the peritoneal tissues taken, MDA(Malonyl-dialdehid), L-6(Interlocking 6) GR (Glutatatyon Reductase), GPX (Glutatyon Peroxidase), TNF-a(Tumor Necrose Factor alpha) were biochemically observed, and the groups were contrasted. Eachperitoneal biopsy tissue was valued histopatologically.Results : Compared with other groups, the biochemical parameters of peritoneal tissue in PPp15group,showed an increase in antioxidane activity along with the ischemic precondition manoeuvre,and less oxidative stress and less inflammation were observed at the end of the experiment.In thesame way, biochemical values in the short-term, in groups Pp10 and Pp15, indicated no oxidativestress, but in the long-term laparoscopic applications a difference between low pressure and highpressure emerged atinitial level.Conclusion : At the beginning of laparoscopic surgery, when laparoscopic ischemic preconditionmanoeuvre is applied, less oxidative stress and less inflammation emerge. In short-termlaparoscopic applications, low pressured pnomoperitoneum has no effect on the decrease ofoxidative stress and the severity of inflammation. In long-term laparoscopic applications, on theother hand, high pressured pnomoperitoneum causes more oxidative stress and more inflammation.
