Antiarrythmic and antiischemic(cardioprotective)effects of remifentanil at sympathetic hyperactivity model in rabbits

Subjects weredivided to the 3 groups (n=6) randomly. In Group 1 10 µmol L-Glutamate was givenintracerebroventricular, to provide the central sympathetic hyperactivity is provided. InGroup 2 40 mg/kg N (omega)-nitro-l-arginine methyl ester was given before İV one hourintracerebroventricular 10 µmol L-Glutamate injection. In Group 3 40 mg/kg N (omega)-nitro-l-arginine methyl ester was given İV before one hour intracerebroventricular 10 µmol L-Glutamate injection. Remifentanil infusion in 6µg/kg/min dose is started 5 minutes before L-Glutamate injection. Heart rate, systolic arterial pressure and mean arterial pressure weremeasured. Heart rate and mean arterial pressure were recorded. During 15 minutessubsequently intracerebroventricular L-Glutamate injection, premature ventricularcomplexes, bigemine ventricular arrhythmias, ventricular tachycardia, ST-segment changesand T wave inversions were recorded.Results: When incidences of Heart rate, rate-pressure product means, prematureventricular complexes and bigemine ventricular arrhythmias were compared between groups,significant differences were not determined. MAP was more significant in group 2 than theother groups (p<0,05). Ventricular tachycardia, ST-segment changes and T wave inversionswere detected significantly low in group 3 (p<0,05).Conclusion: It is found that 5 minute infusion of remifentanil in 6µg/kg/min doseprevents life threatening ventricular tachycardia and myocardial ischemic symptoms, inarrythmia model born of combining of myocardial ischemia and central sympathetichyperactivity in rabbits.ix
