Supply chain is composed of a complex sequence of processing stages, ranging from raw materials supplies to the delivery of end products. In the context of supply chain management, supplier selection is considered as one of the key issues faced by operations and purchasing managers to remain competitive.The objective of this thesis is to propose fuzzy multi-criteria group decision making approaches based on quality function deployment (QFD) for supplier selection. The procedures used in this thesis consider QFD planning as a fuzzy multi-criteria group decision tool and construct two interrelated house of quality matrices to compute the weights of supplier selection criteria and the ratings of suppliers. The first and second developed approaches employ fuzzy weighted average method to calculate the upper and lower bounds of the weights of supplier selection criteria and the ratings of the suppliers. The third proposed approach utilizes the fusion of fuzzy information and the 2-tuple linguistic representation model.In order to illustrate the application of the proposed decision making methods to medical supplier selection problem, a case study conducted in a private hospital is presented. The first two of the proposed methods yield the same ranking. Supplier 1 is determined as the most suitable supplier, which is followed by supplier 7. Using the third proposed algorithm, supplier 7 is determined as the most suitable supplier, which is followed by supplier 1.
