Evaluation of treatment-related endocrinological complications of childhood acute lymphoblastic leukemia patients in remission after treatment

21.In our study, we aimed to evaluate the treatment-related endocrinological complications of acute lymphoblastic leukemia (ALL) patients in remission whose treatment was completed.For this purpose, endocrinological examinations performed routinely in Gaziantep University Medical Faculty Pediatric Hematology and Oncology Outpatient Clinic and recorded in the outpatient clinic files were retrospectively analyzed. The data were obtained by examining the files taken from the archive of Gaziantep University Medical Faculty Hospital. Of all the patients, 19.2% were 5-9 years old, 41.3% were 10-14 years old, 27.8% were ≥15 years, 52.9% were boys and 47.1% were girls. 22.3% of them were in SRG, 60.2% in MRI and 17.5% in HRG risk class. 60.1% of the patients received radiotherapy, 8.7% had short stature, 5.8% had low weight, 11.5% were obese (2.0≤ BMI Z-score <2, 33), 4.8% were found to be morbidly obese (BMI≥2.33). LDL cholesterol in 9 patients (8.7%), total cholesterol in 8 patients (7.8%), triglyceride (for 0-9 years) in 9 patients (29.9%), triglyceride (for ≥10 years) in 17 patients (%) 23.6) high, whereas HDL cholesterol was found to be low in 24 patients (23.3%). HOMA-IR was found to be high in 17.3% of the patients, while 4.8% had metabolic syndrome and 2.9% had subclinical hypothyroidism. 12.62% of the patients (n = 13) had "lower bone mass than expected according to chronological age" (DXA Z-score ≤ -2 SD), 17.52% vitamin D deficiency, 35.05% Vitamin D deficiency was found in 4 of them. While a female patient was diagnosed with precocious puberty, 4 male patients (7.3%) were found to have testicular atrophy consistent with unilateral undescended testis.In conclusion; not only the endocrinological evaluation of patients with acute lymphoblastic leukemia in remission who completed maintenance therapy, but also early diagnosis and treatment of possible side effects are important.Longer-term prospective studies should be planned for the follow-up of endocrinological side effects during the treatment of ALL patients..
