Evaluation of hyperttension in children with unilateral dysplastic, hipoplasic, agenetic and atrophic kidney diseases by ambulatuar blood pressure monitoring

PURPOSE: In this study, it is aimed to diagnose hypertension at children with displastic/agenetic or atrophic kidneys by ambulatuar blood pressure monitor in the early period and take preventive measures, help to protect against severe problems that would arise in the future by starting early treatment. MATERIAL AND METHOD: The study was performed in pediatric nephrology department of İnönü University Medical Faculty between 1995-2013. Totally 49 patients (9 patients with MCDK, 25 with URA, 10 with atrophic kidney, 5 with nephrectomy) at the range of 5-18 ages who have been followed between the years of 1995-2013 and age, sex, weight and heigt matched 30 healthy children between 6-16 ages were included in to the study. Data of age, sex, weight-height measures and BMI were recorded in both groups. Data (files) of the patients were investigated retrospectively. Laboratory parameters and results of radiologic images were recorded. Any blood and urine samples or radiologic images did not taken from children in control group. Blood pressures values those measured with ABPM or manuel manner were assessed and compared. The statistical analysis was made with SPSS 17.0. RESULTS: 22 of the patients were female and 27 of them were male while 15 were female and 15 were male in control group. According to ABPM measures hypertension was detected in 9 of 5 patients with MCDK, 10 of 25 patients with RA, 4 of 10 patients with atrophic kidney and 2 of 5 patients with nephrectomy. Both groups were compared with each other. Sistolic blood pressure values of the patients in MCDK group at nigt and diastolic blood presure values all over 24 hours were higher than conrol group. Sistolic blood pressure values of the patients in RA group at nigt were higher than kontrol group. Diastolic blood presure values all over 24 hours were higher in patients with atrophic and hipoplastic kidney disease than control group. According to measures of ABPM; it is resulted(recorded) that masked hypertension was more common in patient group whether white coat hypertension was more common in control group. CONCLUSION: Incidence of hypertension was statistically higher in patients with dysplastic/agenetic or atrophic kidney diseases. We showed once again that ABPM is effective to detect hypertension in early period to take preventive measures and also to detect masked and white coat hypertension.
