Abstract (EN)

Diabetic nephropathy is the most important cause of chronic kidney disease. Diabetic nephropathy related chronic kidney disease is associated with deterioration in quality of life and reduced life expectancy. Although simple laboratory tests to detect patients with renal dysfunction can be widely used, patients can be diagnosed in the late stages of the disease. Until recently, the only class of drugs shown to slow decline in kidney function were angiotensin converting enzyme inhibitors and angiotensin receptor blockers. İn recent years, as a result of studies conducted with sodium glucose cotransporter 2 inhibitors, it has been determined that they have renal positive effects. In our study, the levels of miRNA let 7a, miRNA 25 and miRNA130b in blood samples taken before and after dapagliflozin treatment were analyzed in patients with diabetic nephropathy. It is thought that if there is a significant difference in the levels of miRNAs investigated before and after treatment, it can be used as a non-invasive biomarker that can be used in the treatment and follow-up of diabetic nephropathy and can be evaluated as a therapeutic target in treatment. In the light of the data obtained at the end of the study, it was determined that there was a significant difference between miRNA let 7a, miRNA 25 and miRNA130b values in the serum of patients before and after dapagliflozin treatment at second month. Although a significant difference was detected between micrototal protein and microalbuminuria levels in the urine before and after dapagliflozin treatment, no significant correlation was found with miRNA levels. More comprehensive studies are needed to better elucidate the positive effect of dapagliflozin use on microalbuminuria in patients with diabetic nephropathy and its relationship with diabetic nephropathy-related miRNA changes.
