Patients who had total knee arthroplasty radiological ipsylateral ankle and clinical evaluation-prospective study

Abstract (EN)

With this prospective study, we aimed to examine the clinical and radiological parameters of the ipsilateral ankle in patients undergoing total knee arthroplasty. 80 knees of 71 patients were examined separately at Afyonkarahisar Health Sciences University Faculty of Medicine. Pre-op and post-op clinical and radiological parameters were examined in patients undergoing total knee arthroplasty (TKA), and findings such as mechanical axis (MA), talar tilt (TT), osteochondral lesion, subcortical cyst, HSS and FAOS scores were recorded. In the study, the average age of the patients was 63.51 and the average post-op follow-up period was 43.98 days. In the study, in which the majority of female patients (83.8%), we found that left knee operations were of light weight (52.5%). A significant improvement in HSS scores was observed in the post-op period compared to the pre-op period (pre-op: 63.6±9.02; post-op: 79.55±8.72). There were also improvements in symptoms, pain, daily life activities (DIA), sports and recreational activities, and quality of life scores in the pre-op and post-op period. The pre-op MA value was in the range of 9.06-15.05 and the average was 11.62±11.65. The post-op MA value was in the range of 1.26-7.27 and the average was 4.29±4.2. The pre-op TT value was in the range of -1.05-1.13 and the average was -0.02±0.34. Post-op TT is in the range of -1.4-1.11 and the average is -0.2±0.1. Osteochondral lesion was detected in talus region in 36.3% (n=29) of patients, tibia region in 10% (n=8) and subcortical cyst subtalar region in 10% (n=8) of patients during preoperative period. In the post-op period, osteochondral lesion was seen in talus region in 47.5% (n=38) of patients, subcortical cyst was seen in subtalar region in 15% (n=12) of tibia region and in 17.5% (n=14) of patients. In the control group, 48.8% of patients (n=39) had an osteochondral lesion in the talus region, 22.5% (n=18) had a subcortical cyst in the tibia region and 18.8% (n=15) had a subcortical cyst in the subtalar region. There was no statistically significant difference in the comparison of pre-op and post-op pain, DIA, sports and entertainment, quality of life according to the areas of lesion presence compared to the state of new lesion development in post-op. As a result: We think that the operation was generally successful as a result of the observed improvements in symptoms, pain and mechanical axis parameters in the pre-op and post-op period. However, we think that the increase in osteochondral lesions and subcortical cysts that occur in the ankle in the post-op period, especially the biomechanical balance, should be monitored more closely during the recovery process.
