Comparison of postoperative neutrophyl / lymphocyte ratio in general and spinal anesthesia in TURP patients

Abstract (EN)
Objective: We aimed to investigate the effects of general and spinal anesthesia on neutrophil / lymphocyte, platelet / lymphocyte ratio, CRP, MPV levels, which are easy and effective indicators of inflammatory response in TUR-P patients. Material-Method: Fifty ASA class I-III patients, aged 35-80 years, who underwent elective TUR-P were included in our study. The patients were randomly divided into 2 groups; general anesthesia group (Group G, n = 25) and spinal anesthesia group (Group S, n = 25). After routine general anesthesia was applied to the patients in Group G, after the block levels of the patients in Group S were controlled, surgery was initiated when the sufficient anesthesia level was reached. Hemodynamic data of the patients were recorded during the operation. Preoperative (T1) and postoperative 24hour (T2) blood samples were obtained from all patients to evaluate the stress and inflammatory response. Samples were sent to the Clinical Biochemistry laboratory immediately after collection. CRP and hemogram parameters were seen in blood samples. Neutrophil / lymphocyte, platelet / lymphocyte, and MPV levels were recorded among the hemogram parameters. Results: The patient groups included in our study were similar in terms of demographic data. No statistically significant difference was found between postoperative spinal and general anesthesia in terms of neutrophil / lymphocyte, MPV, monocyte, CRP values between the groups. In the general anesthesia group, a higher postoperative platelet / lymphocyte ratio was found. In the intergroup comparison, postoperative neutrophil / lymphocyte, platelet / lymphocyte values were found to be significantly higher than preoperative values in the general anesthesia group. Again, in the within-group comparison, a higher CRP value was obtained postoperatively in the spinal anesthesia group. In the time intervals when vital signs were recorded, the pulse rate was lower in the group that was administered general anesthesia during the T2 (post-administration) period, higher SpO2 was recorded in the general anesthesia group at T4 (20 min), T5 (30 min) T6 (end of surgery) time. Conclusions: In this study, neutrophil / lymphocyte ratio and CRP values were examined to investigate the effect of spinal anesthesia and general anesthesia applied to patients who underwent TUR-P on the inflammatory response, and no significant difference was found between the groups. However, in the comparison made within the group, a higher postoperative neutrophil / lymphocyte ratio was found in the general anesthesia group. These different results found in the studies show that further studies with larger number of patients are needed to fully clarify the effects of the anesthesia method on the inflammatory response.
