Abstract (EN)

Introduction and Aim: Among the upper extremity blocks performed under the guidance uf USG, the correlations between perfusion index increase rates of supraclavicular and infraclavicular blocks motor and sensory block times, and perfusion index were examined and it was aimed to investigate whether the difference in perfusion index between the two blocks had an effect on anesthesia. Material And Method: In our study, 74 patients between the ages of 18-75 who were to undergo arm and forearm surgery and were in the ASA I-II-III risk group were included. They were divided into two main groups as I and II, 25 ml 0.5% bupivacaine + 5 ml 2% lidocaine administered by the responsible anesthesiologist were included in Group I, patients who underwent supraclavicular block were included in Group II. Perfusion index values at 0., 3., 5., 10., 20., 30., minutes and PACU were recorded. Motor and sensory examinations were performed with Modified Bromage Scale and Pin prick tests. Preoperative and intra operative blood pressure, pulse, SPO2 values recorded. Results: In our study, no significant change was found in hemodynamic data in both groups after the block. It was observed that there was a significant decrease in blood pressure at the 5th and 10th minutes, but no difference was founda between the two groups. Significant increases were observed in the perfusion indices up to the pacu at all times, more in group I than in group II. These rates of increase were found to be compatible with sensory block time in group II and motor block time in group I.Perfusion index increase rates were found to be equal in both blocks of Pacu. Conclusion: In patients who will undergo arm and forearm surgery in emergency and elective v conditions and will be operated on, we determined the success of the block earlier with the Perfusion index in supraclavicular and infraclavicular block compared to traditional methods. And we concluded that vasodilation due to sympathectomy can be measured objectively with the perfusion index, that it can be used in suitable patients without disturbing hemodynamics in two blocks, and that the contribution to tissue oxygenation is the same in both blocks since the post-op perfusion index increase rates are equal
