Abstract (EN)

Aim: Crimean-Congo Hemorrhagic Fever (CCHF) is an endemic infectious disease in Tokat region, can be mortal, and causes tick-borne viral hemorrhagic fever. The aim of this study is to examine the factors associated with prognosis in patients with CCHF in Tokat Gaziosmanpaşa University Medical Faculty Hospital. Material and Method: In this study, clinical datas of 281 patients with CCHF treated at Infection Clinic of the Faculty Hospital between the years 2018 and 2021 were evaluated, retrospectively. The general characteristics of the patients, factors related to mortality, factors associated with intensive care hospitalization and the factors affecting the length of hospitalization were examined. Results: Muscle -joint pain, loss of appetite, vomiting (p<0,005), and nausea – diarrhea (p<0,001), are the symptoms related to the mortality. The cases having laboratory results of PLT ≤26000/mm3, AST ≥342 IU/L, LDH ≥1019 U/L, CRP 40 µg/ml, ferritin ≥20000 ng/ml, aPTT ≥59 sn, INR ≥1,6, fibrinogen≤ 113 mg/dl, D-dimer ≥5,59 µg/ml, vitamin D ≤ 8,81 ng/ml had high mortality risk. In additıon to the symptoms related to mortality, headaches, loss of appetite, bleeding were the findings led to the patients to the intensive care units. The cases having PLT ≤26000/mm3, AST ≥342 IU/L, ALT ≥579 IU/L, LDH ≥1019 U/L, CK ≥357 IU/L, CRP ≥35 mg/l, procalsitonin ≥0.449 ng/ml, IL-6 ≥30 pg/ml, ferritin ≥20000 ng/ml, aPTT ≥59 sn, INR ≥1.33, fibrinogen≤ 119 mg/dl, D-dimer ≥8,49 µg/ml, vitamin D ≤ 8,81 ng/ml had the high risk of requring intensive care treatment. During the follow up process the measured highest CRP, Procalsitonin, INR, aPTT, IL-6, D-dimer values together with the lowest value of PLT and fibrogen were the statistically significant variables which increased the patient‟s duration of hospitalization. Conclusion: In order to predict the course of the illness of CCHF patients treated in the clinics, all symptoms must be examined dexteriously, with everday testings of hemogramme, AST, ALT, CK, LDH, INR, APTT, D-dimer, fibrogen, ferritin analysis must be conducted, also vitamin D must be supplied to the cases having deficiency of vitamin D.
