Protecting and repairing the meniscus is very important to provide knee biomechanics and functionality. Therefore, many new biological techniques have been defined to increase the healing rate of tears after meniscus repair. İn our study, we compared the clinical and functional results, surgical success and factors affecting these at 18 months of patients who in the treatment of vertical / longitudinal meniscus tears, bone marrow stem cells, blood and other tissue healing factors are revealed by drilling the femur intercondylar notch (BMVP), anterior cruciate ligament reconstruction (ACLR) with meniscus repair and isolated meniscus repair. Between 2015 and 2019, 83 patients who applied to Gaziantep University Hospital Orthopedics and Traumatology Clinic with complaints of knee pain, instability and who were diagnosed with vertical / longitudinal meniscus tear and underwent arthroscopic repair (30 with isolated repair, 28 with ACBR repair and 25 with BMVP). We analyzed the data retrospectively. We evaluated the clinical and functional conditions of the patients with Lysholm, WOMAC, IKDC and VAS before and 18 months after surgery. We compared surgical success with Barrett's criteria. We analyzed the causes of failure according to demographic data of the patients, smoking, injury time, location, shape and zone of the tear, type and number of sutures. Of the patients included in the study, 58 were male, 25 were female, 61 were medial meniscus and 22 were lateral meniscus lesions, 51 had simple longitudinal tears, 18 had a bucket handle, 14 had a kompleks tear, 68 patients had a ruptured red-white zone, 15 was in the red-red zone and 11 anterior 26 korpus and 46 posterior tears in all of them. 42 patients were smoking, 41 were non-smokers, 49 were BMI <25 and 34 were BMI> 25. The mean age of the patients was 29.73 ± 9.31 and the average sütürnumber was 3.36 ± 1.28. When Lysholm WOMAC and IKDC values were compared before and 18 months after surgery, a significant improvement was achieved VIII in three groups (p: 0.001). İn 18th month values, we found that the clinical and functional results of the patients who underwent meniscus repair with BMVP and ACBR were similar (Lysholm p: 0.951, WOMAC p: 0.241, IKDC p: 0.984). Both of these procedures had better clinical and functional results compared to patients with isolated meniscus repair (Lysholm p: 0.001 WOMAC p: 0.027 IKDC p: 0.001). In addition, treatment failed in 15 patients according to Barrett's criteria. We found that the zone of tear (p: 0.044) and kompleksity (p: 0.001), BMI> 25 (p: 0.003) and smoking (p: 0.049) had negative effects on failure. Better clinical and functional results after meniscus repair with BMVP and ACBR compared to isolated meniscus shows us that bone marrow stem cells, blood components and tissue healing factors migrating into the joint have positive effects on meniscus repair and knee functions. In addition, we believe that smoking has negative effects on the healing of the tear with the high BMI of the patients, the kompleks and the red-white zone of the tear. In the repair of vertical / longitudinal meniscus tears, we recommend the routine application of BMVP, which can be applied easily, which increases knee functions.
