Effect of body mass index on radical cystectomy surgery

Introductio Obesity is a growing health problem in the world. In this study, effects on perioperative and postoperative outcomes of the body mass index of the patients who underwent radical cystectomy operation were evaluated. Material and Method 82 patients who treated with open radical cystectomy (ORC) or laparoscopic radical cystectomy (LRC) at our clinic between the dates of October 2011 - February 2016 due to bladder cancer, are divided into 3 groups with regard to BMI<25, 25≤BMI<30 and 30≤BMI. Data held in oncological index were retrospectively evaluated. After the initial tests; physical examinations and biochemical tests on follow-ups; PET-CT imaging and computerized tomography were made to the patients with local or distant metastases suspected. In the study, exclusion criteria handled as the reason of the cystectomy done is not the bladder cancer, and 1 patients were excluded from the study for this reason. Findings 31 males and 1 female in the group of BMI<25, 24 males and 3 females in the group of 25≤BMI<30, 17 males and 6 females in the group of 30≤BMI have detected. Considering the age range of patients, it was seen that; 59,6(48-64) in the group of BMI<25, 65,3(50-78) in the group of 25≤BMI<30 and 62,9(41-79) in the group of 30≤BMI. With regard to duration of the operation, it is seen that; 374,6(200-650) minute in the group of BMI<25, 359,2(250-600) minute in the group of 25≤BMI<30 and 395,6(340-600) minute in the group of 30≤BMI. It was detected that; patients who received postoperative chemotherapy were found to be 17(% 53,1), and patients who have not received postoperative chemotherapy were found to be 15(% 46,9) in the group of BMI<25, patients who received postoperative chemotherapy were found to be 7(% 25,9), and patients who have not received postoperative chemotherapy were found to be 20(% 74,1) in the group of 25≤BMI<30, and patients who received postoperative chemotherapy were found to be 6(% 26,1), and patients who have not received postoperative chemotherapy were found to be 17(% 73,9) in the group of 30≤BMI. Findings mentioned above have found statistically significant, however significant different have not found in other parameters in association with BMI. Conclusion In our study, it was found to be the operation period of obese patients is significantly longer. Although, not statistically significant, postoperative complications, length of hospital stays, cost per patient was found to be higher in obese patients. With regards to oncologic outcomes; no difference was detected between obese patients and overweight patients or normal weight patients. This result shows that; open or laparoscopic radical cystectomy can be safely recommended to obese patients. Keywords: Obesity, cystectomy, body mass index
