Investigation of the relationship between systemic immune inflammation index and non-dipper hypertension in newly diagnosed hypertension patients

Introductıon: Hypertension (HT) is the most common chronic disease worldwide and has a significant impact on public health. In the classification made by ambulatory blood pressure measurement, dipper hypertension is defined as a decrease of 10% or more in blood pressure values evaluated during the night compared to the values measured during the daytime and a minimum decrease of 10% is defined as non-dipper hypertension. It has been determined that there is an increase in mortality and morbidity rates due to cardiovascular diseases, which are observed more frequently in individuals with non-dipper hypertension. In this study, it was tried to investigate whether there is a relationship between the systemic immune-inflammation index (SII), which has been shown in many studies to be significantly related to important cardiovascular diseases such as hypertension, coronary artery diseases and heart failure, and non-dipper hypertension. Materials and Methods: Patients who applied to the cardiology outpatient clinic between January 2022 to April 2022 with newly diagnosed HT were included in the study consecutively. Twenty-four hour ambulatory blood pressure measurement was performed for each patient, and the study population was divided into three groups as dipper or non dipper hypertension (according to whether there was a ≥10% reduction in nighttime blood pressure values) and normotensive control groups. The patients were evaluated with their demographic characteristics, comorbidities and laboratory values. Statistical analyzes were performed using the SPSS system, p<0.05 was considered statistically significant. Results: There was no significant difference between the groups in demographic parameters. In the echocardiographic evaluation, it was determined that the left ventricular wall was thicker in the hypertensive group compared to the control group (p=0.017). While the non-dipper group had higher SII levels compared to both dipper HT and control groups in (p<0.001),there was no significant difference between dipper HT and control groups in SII levels (p>0.05) .Serum high sensitive C Reaktive Protein (hs-CRP ) levels were significantly higher in the non-dipper group than in the other groups (p=0.018). After multiple linear regression analysis; it was observed that SII parameter was predıct non-dipper hypertansion independently. Conclusion: It has been demonstrated that SII parameter, which can be simply calculated with the data obtained from the complete blood count and do not require additional costs, is associated with non-dipper hypertension. However, this relationship needs to be confirmed by future studies with larger populations and prospective studies in this area.
