An ethnographic study on the breastfeeding experience of mothers with premature babies in the adaptation room after the neonatal intensive care unit

Abstract (EN)
Aim: This study was carried out to ethnographically evaluate the breastfeeding experiences of mothers with preterm infants in the transition room after discharge from the neonatal intensive care unit. Method: This research was conducted using a qualitative research design and employed the rapid ethnography method. Data were collected through in-depth individual interviews and non-participant observations conducted with 18 mothers of preterm infants in the transition rooms of Ankara Etlik City Hospital's Maternity Department. Thematic analysis was carried out using Braun and Clarke's framework. Results: The infants breastfed in the transitional care room were born between 32 and 36 weeks of gestation. As a result of the thematic analysis of in-depth interviews conducted with 18 participants, five main themes and fifteen sub-themes reflecting their breastfeeding experiences were identified. In the theme The Meaning of Breastfeeding: Motherhood, mothers described breastfeeding not only as a means of nutrition but also as an essential part of their maternal role and a tool for bonding. In the theme Emotional Experiences During Breastfeeding, mothers reported intense emotions such as anxiety, fear, guilt, and exhaustion. In the theme Societal Expectations and Cultural Norms, social pressure, religious-cultural beliefs, and traditional expectations were found to influence breastfeeding decisions. In the theme Challenges and Coping During the Breastfeeding Process, physiological difficulties due to prematurity, insufficient milk supply, and lack of support made the process more difficult. In the theme Facilitators of the Breastfeeding Process, factors such as family support, guidance from healthcare professionals, and physical contact with the baby positively influenced the breastfeeding experience. Conclusion: This study, conducted in the transitional care rooms after NICU discharge, shows that breastfeeding is not merely a physical act, but a multidimensional experience shaped by cultural values, maternal identity, and emotional well-being. The transitional care period represents not only medical follow-up, but also a cultural and emotional transition into motherhood. Culturally sensitive nursing practices support mothers in navigating this process more effectively and meaningfully. Keywords: Preterm infant, transition room, ethnography, cultural-nursing
