The relationship between prognosis of expression levels of components in microrna biogenesis in patients with traumatic brain injury in intensive care

Abstract (EN)

A definitive prognostic marker has not been found yet, although clinical, radiological and electrophysiological data contribute to the prognosis of traumatic brain injury patients. MicroRNAs are small, endothelial, non-coded RNA molecules consisting of 19-28 nucleotides that regulate protein synthesis at the post-inflammatory level. The miRNA biogenesis takes place in two steps: the nucleus and the cytoplasm. Specialized molecules serve in these steps. The most specific of these are the Drosha enzyme from the RNase III family and the cofactor Pasha (DGCR8), the transport receptor; Exportin5, the Dicer endonuclease, an enzyme belonging to the RNase III family; the Argonaute protein that provides stable chain selection. It is known that if the molecules involved in microRNA synthesis are not able to function correctly, the function of the targeted microRNA is impaired. In recent years microRNAs have been investigated as biomarkers in various diseases. Clinical and experimental studies have shown many microRNAs that are valuable in terms of prognostic and diagnostic levels, which are measured in traumatic brain injury. However, data on the levels of microRNA biogenesis components are limited in patients with traumatic brain injury. In our study, it was planned to measure Dicer, Drosha, DGCR8, Exportin5 and Argonaute2 levels of 21 patients who were admitted to Akdeniz University Anesthesia Intensive Care Units and who had traumatic brain injury in the 4-6. hours and 72 hours after the trauma and compared with the control group. The control group of the study consisted of 21 patients who were followed up in the intensive care unit due to the trauma but were not diagnosed with brain injury. It is aimed to determine whether the protein levels measured in early (post-traumatic 4-6. hours) and late (72 hours) period are different and to determine the effect of this change on prognosis. According to the results of our study, the expression changes of Dicer, Drosha, Exportin5, DGCR8 and Argonaute2 proteins from miRNA biogenesis components were not found statistically significant in patients with severe TBI. However, when we investigate the patients one by one, it was noticed that Dicer expression of the patients who increased from severe TBI to mild TBI class increased and that the expression of Ago2 decreased in most patients in this group. It was observed that Dicer expression profile was increased in patients who were discharged from the intensive care unit in a short time.
