Effect of platelet rich plasma (PRP) and stromal vascular fraction (SVF) supplemented with stem cells and niches on sperms damaged in experimental diabetic model with high glucose medium

Aim: Diabetic damage (DD) was created in this work by utilizing a high glucose medium in the culture media of healthy animals sperm. We used mesenchymal stem cells (MSC), conditioned media (CM) Niche, which is the factors they secrete for 48 hours, and Stromal Vascular Fraction (SVF) mixed with Platelet Rich Plasma (PRP) to minimize sperm damage in experimental diabetes. Sperm samples from 15 Wistar Albino rats were collected and divided into 15 groups in 6-well flasks filled with culture medium. Materials and Methods: The groups were divided into Control (CG), Sham (SG), DD, DD+ MSC, DD + CM, DD+MKH+ CM, DD+ PRP, DD+ SVF, DD+ MSC+PRP, DD+ MSC+ SVF, DD+ CM + PRP, DD+ CM + SVF, DD+MKH+ CM + PRP, DD+ MSC + CM + SVF, and DD+ MSC+ CM + PRP+SVF. Smears of sperms were prepared after analyzing morphology, viability, and movement in three-hour application by phase-contrast microscopy. Smears were used for morphology and immunocytochemistry which e-NOS and i-NOS for oxidative stress, and TUNEL, Caspase 3, 8, and 9 for apoptosis were used. It was observed that morphological, viability, and movement abnormalities produced by diabetic medium damage to sperm were linked to higher levels of eNOS and iNOS, as well as higher levels of apoptotic markers. The applications reversed these markers to lower level while preserving sperm morphology, vitality, and motility. These results were statistically significant. While individual applications showed promise, the combination of MSC+CM+PRP SVF showed much more success. Results: The high glucose medium as an in vitro model for DD has been proven to induce sperm damage similar to diabetic testis which could be good model for other treatment options. As a consequence of raising the number of MSC, boosting the growth factors, and enhancing the signaling mechanisms by combination of cell therapies used in this study, it was found that the combination of MSC, PRP, CM, and SVF might be highly effective as a cellular therapy. Conclusions: Our results, which will aid in the creation of a novel low-cost, high-efficiency regenerative treatment, such as cellular therapy for male infertility which has a high prevalence in society and is gradually expanding, will provide hope to families who wish to have a baby. Experimental animal and human investigations will be used to confirm these findings and elucidate the mechanisms, allowing them to be used in the clinic.
