Objective: This study aimed to evaluate the relationship between the development of contrast-induced nephropathy and triglyceride-glucose index (TyG index) in patients who underwent coronary angiography due to acute coronary syndrome at Zonguldak Bülent Ecevit University Hospital. Materials and Methods: Clinical and biochemical factors potentially influencing CIN were analyzed. The study included 238 patients diagnosed with acute coronary syndrome who underwent coronary angiography. All cases were included in the analysis. CIN development was defined as the dependent variable, while age, sex, percentage change in glomerular filtration rate (GFR), C-reactive protein (CRP), hemoglobin levels, contrast volume administered during the procedure, and TyG index were included as independent variables Findings: The mean age of participants was in the middle-to-advanced range, and males were predominant. Patients who developed CIN had significantly higher mean contrast volumes and CRP levels, while the percentage change in GFR was significantly lower. Binary logistic regression analysis indicated that age (p = 0.015), GFR change (p < 0.001), and contrast volume (p = 0.014) were significant predictors of CIN development. Each one-year increase in age increased the likelihood of CIN by 4.7%, and each 1 mL increase in contrast volume raised the CIN risk by 1.1%. In contrast, sex (p = 0.359), CRP (p = 0.223), hemoglobin (p = 0.210), and TyG index (p = 0.590) were not statistically significant. The model's overall accuracy was 92.0% . Results: The findings suggest that advanced age, decreased GFR and higher contrast volume are independent risk factors for CIN, whereas the TyG index does not have a significant predictive role. The model demonstrates high accuracy in predicting CIN, emphasizing the importance of early risk assessment and careful contrast use in vulnerable patients.
