Abstract (EN)
Introduction and Objective: In this study, it was aimed to retrospectively investigate the patients who had been diagnosed with blunt liver trauma who admitted to our clinic between December 2006 and December 2016; and to assess our demographic, clinical and laboratory experience by observing the changes in the diagnosis, follow-up and treatment process that we applied in organ injuries. Material and Method: Retrospectively 200 patients, who had been admitted to Dicle University, Faculty of Medicine Pediatric Surgery due to Liver trauma were studied and included in the study. In addition to demographic characteristics such as age and gender of the patients, pre-treatment injury mechanism, degree of injury with radiological methods (MediCalc® - U.S. trauma, organ severity scoring system), treatment modality, time of hospital stay in intensive care unit, comorbid injuries and mortality rates, and Pediatric trauma scores were evaluated. (Frequency, Percent, Mean, Standard deviation) as well as descriptive statistical Kolmogorov Smirnov distribution test was used to examine the normal distribution. Results: Of the pediatric patients, 73,2% (139) was male, and 26,8% (51) was female. It was determined that 49,5% (94) of the pediatric patients was in the 4-8 age group, 22,1% (42) was in the 9-12 age group, and 21,6% (41) was in the 0-3 age group. It was determined that 57,6% of the presenting symptoms to the hospital was falling from a higher place, 39,5% was non-vehicular traffic accidents, 7% was the stab injuries, 2,6% was vehicular traffic accidents to fallen object. It was found that 73,2% of the patients had not undergone blood transfusion and 26,8% had undergone blood transfusion. It was also found that 4,2% of the patients had complications and 94,7% had no complications. It was also observed that 1,1% (2) of 190 patients was ex. When the AAST classifications were examined; 25,8% did not have a grade. Of the patients, 23,7% was Grade 3, 21,1% was Grade 4, 15,3% was Grade 2 and 10,5% was Grade 1. It was investigated whether there was a significant difference between the hospitalization and the discharge average for ALT and AST values, among the biochemical values of all children included in the study. Accordingly, there was a statistically significant difference between ALT admission and ALT discharge (p<0.001). The ALT admission average was 393.12, whereas the ALT discharge was dropped to 172.14. On the other hand, a statistically significant difference was also found between AST admission and AST discharge (p<0.001). Although the AST admission average was 582.55, whereas the AST discharge was dropped to 179.89. The relationship between the type of injury and age was investigated using Chi-square test. According to the test results, it was found that there was a statistically significant difference in the way of injury in terms of the age of the children (p<0.05). Accordingly, there is a decrease in the total number of injuries as the age increases in general. The highest number of injuries was seen between the ages of 4-8, whereas the minimum number of injuries were observed between 13 and 17 years of age. Similarly, falling from a higher place and non-vehicular traffic accidents were mostly seen between the ages of 4-8. A relationship between the children who underwent computed tomography and those who underwent ultrasonography was also investigated. According to this, there was a statistically significant relationship between abdominal USG and CT (p<0.05). Of the patients who did not undergo abdominal CT, 93.2% was found to undergo USG. The relationship between AAST and quadrant fluids was investigated by the Chi-square test. According to the test result, AAST showed a statistically significant difference according to the quadrant fluids of children (p<0.05). Conclusion: In the pediatric age group, the treatment of traumas resulting from falling from high, traffic accidents and falling objects is possible with conservative and surgical treatment in general. Compared to the adult age group, morbidity and mortality is lower thanks to smaller body surface and flexible skeletal system. Since children are mostly exposed to falling from higher placed, it is necessary to educate their families and prepare safer gaming environments in order to minimize morbidity and mortality due to possible trauma.
