Abstract (EN)

Acute pancreatitis is an inflammatory disease of the pancreas with a rapid onset and multiple etiologies. The most common causes are alcohol and cholelithiasis. For the diagnosis of acute pancreatitis, one of three criteria must be met. These are; upper quadrant pain radiating to the back, serum amylase or lipase at least three times above the upper limit of normal, and imaging (contrast-enhanced CT, contrast-enhanced MRI, abdominal USG) compatible with acute pancreatitis. The worldwide prevalence of acute pancreatitis varies between approximately 4.6 and 100 per 100,000. In general, it has a global incidence of 30-40 cases per 100,000 population per year. Gallstone etiology is higher in southern European countries such as Türkiye. It is not clear what triggers inflammation in the pancreas. The basic theory focuses on the activation and release of trypsinogen and other enzymes from the pancreatic acini, which results in autodigestion of the pancreas and a systemic inflammatory response. In recent years, it has been suggested that acute pancreatitis is caused by the activation of digestive proteases in the pancreatic acinar cells. Carboxypeptidase B1 activates trypsinogens. The activated trypsinogen is converted to trypsin, initiating the inflammatory process that causes acute pancreatitis. Various scores are used to assess the severity and prognosis of acute pancreatitis. Clinical and laboratory parameters are evaluated with these scores. Today, replacement and analgesic treatments are applied as treatments. In our study, we aimed to investigate the diagnostic efficacy and prognostic value of Carboxypeptidase B1. As a result of our study, we found that the diagnostic efficacy of Carboxypeptidase B1 was low.
