Clinical study for hormonal variations and their association with CFTR , FSHR genes expression in male infertility

Abstract (EN)

Current study is conducted for the period from September 2022 until June 2023 at infertility Unit, Al-Nasiriyah Teaching Hospital, Thi-Qar, Iraq. To examine masculinity and marital infertility , the clinical cases of the patients were collected after being nominated by the urologist, this study aims to evaluate levels of total Follicle-stimulating hormone (FSH), Luteinizing hormone (LH), and Testosterone (T) their relationship to male infertility and investigate the combined effect of FSHR and CFTR gene polymorphism and infertile in case–study men. The present study includes number of 150 samples their age ranging from 20-40 years old , were divided into two groups, first group (healthy group) includes 75 men and patient group include a number of 75 infertile men each group were divided into four groups according the age categories. Present study showed that mean age for patients is 29.28 ± 6.38 years. The results also revealed that the most patients in age groups 26-30 years of age accounted for 28 (37.3 %), present study indicate highly significant increase of median levels of serum FSH in patients with infertility in comparison with healthy control, 3.43 (3.77) versus 2.18 (1.95) respectively, and the difference was highly significant (P < 0.001). Also, the median levels of serum Luteinizing Hormone (LH) was increased in patients with infertility in comparison with healthy control, 5.64 (3.76) versus 3.35 (1.86) respectively, and the difference was highly significant (P < 0.001). But the median levels of serum testosterone hormone lower in patients with infertility in comparison with healthy control, 3.82 (2.85) versus 5.42 (1.55) respectively, and the difference was highly significant (P < 0.001), Testosterone hormone test shows higher sensitivity (81%) while FSH (72%) and LH (78%). Regarding CFTR ΔF508 (C/T) gene in patients with infertility. Although there is no significant difference in the frequency distribution of genotypes between cases and healthy control groups. But there was increased risk of infertility with genotype TT versus CC as Odd ratio 1.54 was TT, with statistically significant and may be considered as etiological fraction for infertility. Also there was increased risk of infertility with genotype CT versus CC as Odd ratio was 1.95, and may be considered as etiological fraction for infertility. Concerning FSHR (rs1394205) G/A gene in patients with infertility. In co-dominant mode, there was highly significant difference in the frequency distribution of genotypes between patients and control groups (P = 0.001). There was increased risk of infertility with genotype AA versus GG as Odd ratio was 6.26, with statistically significant (P=0.001), and may be considered as etiological fraction for infertility, also there was increased risk of infertility with genotype GA versus GG as Odd ratio was 2.35, with statistically significant (P=0.001), and may be considered as etiological fraction for infertility. The current results show significant association between CFTR ΔF508 (C/T) genotype serum FSH levels, were the FSH levels increased in TT genotype in compared to CC and CT genotype respectively. But the present results show non-significant association between CFTR ΔF508 (C/T) genotype serum LH levels. Whereas the present results show non-significant decreased the serum Testosterone Hormone levels in TT genotype in compared to CC genotype and CT genotype respectively. The present investigation show non-significant association between FSHR (rs1394205) G/A genotype and serum hormones level. Although increased the serum FSH levels and serum LH levels in AA genotype in compared to GG and GA genotype respectively. But the present results show non-significant decreased the serum Testosterone Hormone levels in AA genotype in compared to GG genotype and GA genotype respectively.
