Abstract (EN)
Upper gastrointestinal tract (GI) bleeding is a frequent cause of mortality and morbidity. In this study, all-cause mortality risk at hospital, in the 6-month period after admission and in the 1-year period and, rebleeding risk were evaluated using the Crusade, Rockall and, Glaskow-Blatchford scoring systems. This study was carried out among patients attended to Tokat Gaziosmanpasa University Hospital with the diagnosis of upper GI bleeding between 2019 and 31 December 2021 retrospectively. Demographic and clinical characteristics of the patients were recorded. Data of the patients were recorded including age, gender, comorbid illness presence and count, endoscopic findings, cardiac illness findings, peripheric arterial disorder findings, mortality status in 1 year of bleeding episode, rebleeding status in 1 year of bleeding episode and vital signs (blood pressure, pulse) were recorded. Laboratory parameters on admission including hemoglobin, hematocrit, leukocyte count, platelet count, BUN, creatinine, calcium, sodium, chlorine, potassium, ALT, AST, total bilirubin, direct bilirubin, LDL, triglyceride, and INR were also recorded. A total of 194 people were included in the study. The mean age of the patients was 66,3(±17,15) and, 67.5% (n=131) of whom were male. Comorbid illness was found in more than half of the patients. Most frequent comorbid illnesses were hypertension and coronary arterial disease, respectively. Diabetes mellitus and Alzheimer's disease were comorbid illnesses that were related the mortality. It was found that mortality increased with increasing age. Endoscopic treatment was applied to all patients, rebleeding occured in 60 patients (30.9%) during follow-up, and mortality was detected in 45 (23.1%) patients. It has been shown that while the Rockall score is more effective to demostrate hospital mortality, the Glascow-Blatchford score is more effective demonstrating 1-year mortality compared to other scoring systems. It is found that all of the scoring systems including Rockall score, Glasgow-Blatchford score and Crusade score are effective in predicting rebleeding risk.
