Are high levels of serum B2 mikroglobuline, independent inflammatory markers from renal dysfunction in geriatric population?

ABSTRACT Are High Levels of Serum B2 Mikroglobuline, Independent Inflammatory Markers From Renal Dysfunction In Geriatric Population? Object: This study was to aim to determine the microglobulin level of serum B2 in geriatric patient population as an inflammatory marker and to compare with other inflammatory markers in order to determine its effectiveness, reliability and whether it is brittleness marker or not. Materials And Methods: In order to evaluate the relationship between inflammation and Serum B2 microglobulin level, totally 81 participants were attended as 20 patients with rheumatoid arthritis (chronic inflammatory disease), 20 patients with osteoarthritis (chronic non-inflammatory disease), 22 patients with acute infection and 19 control groups( healthy individuals). Malignancy, grade 3-4 congestive heart failure, renal disfunction, chronic liver disease, obesity, autoimmune disease, anti-inflammatory, antibiotic and immunosuppressive drug therapy which affect the inflammatory markers and people who are except from geriatric population and under 65 years of age could not have attended to this study. Muscoloskeletal and systemic examens of all groups have been done. Serum B2 was compared with other inflammatory makers such as sedime, CRP (c-reactive protein), procalcitonin, ferritine, IL-1, IL-6, TNF-alpha, D vitamine and routine biochemical tests. To evaluate the touchiness, Quality of Life Measure and Clinical Frailty Scale(cfs) are applied. Symptoms: 29 (35.8 %) men and 52(64.2%) women patients have attended. Average of B2 microglobin level of all groups have been determined as 3.8±1.3 mg/L (1.24-7.9 mg/L). Significant difference was determined as statistically between groups and B2 Microglobulin level. B2 microglobulin was determined as high in both acute infection and rheumatoid arthritis. According to the quality of CFS and SF36; Score of quality of life is the lowest in acute infections. And increased values have been seen in Rheumatoid arthritis, osteoarthritis and healthy individuals. We have determined indirectly related between Serum B2 Microglobulin level and quality of life. B2 Microglobulin has positive correlation with age, CRP, sedime, procalcitonin, WBC, ferritin, HES, IL-6, TNF alpha, CFS. Otherwise, it has negative correlation with hemoglobin, htc, Fe/TİBC (TSAT), D-vitamine, HDL, albumen and SF-36. In the case of acute infectious, the level of D vitamine, T.cholesterol, HDL, LDL and TG are decreased. Result: B2 Microglobin level is valuable as scores of SF36 and CFS in touchiness evaluation. Quality of life of patient decreases while their B2 Microglobulin level increases. Beta-2 Microglobulin level is independent from other variables and it has shown positive correlation with age, IL-6, TNF-alfa and CFS scores and it has shown negative correlation with SF-36 score.cSerum Beta-2 Mikroglobulin level has shown that it has correlation with CRP in the case of acute infectious. Serum Beta-2 Microglobulin level can be inflammatory and touchiness maker.
