Integration of failure mode and effect analysis with grey relational analysis - application attempt in an enterprise

Agricultural production is increasing in parallel with the increase in world population. However, agricultural production cannot meet the existing demand. Therefore, the companies desire to increase the yield from agricultural products and to grow market shares increases the competition and makes fertilizer production even more important. In this direction, the thesis was set out in order to prioritize and provide corrective advice with the help of Failure Modes and Effects Analysis, encountered failure modes in the production process of a fertilizer producing enterprise in Turkey. As a result of the application of the Failure Modes and Effects Analysis, the same Risk Priority Number values can be obtained although the failure risks are very different from each other. This situation is a deficiency and criticized feature of the Failure Modes and Effects Analysis in the literature and may prevent the prioritization from being healthy. In order to overcome this deficiency and prioritization to be healthier, Grey Relational Analysis method, Failure Modes, and Effects Analysis were applied together to help decision making when there is no definite information. This is thought to contribute to the enterprise in terms of preventing potential failures before they reach the customer. As a result of the Grey Relational Analysis, the relationship statuses of the prioritized failure modes, and the causes of failure were tried to be determined by means of the graphs created in a two dimensional space by the Correspondence Analysis and it was recommended that the enterprise concentrate on these causes of failure.
