Abstract (EN)
ÖZET (İngilizce) Demir, İH. (2018). Determination of the effect of music on pain, anxiety and physiological signs during the intravenous administration of 4-6 years old children who applied to the hematology and oncology daily diagnostic and treatment center. Yeditepe University Institute of Health Sciences, Department of Nursing, Master Thesis. Istanbul. Introduction: Most children are negatively affected by medical procedures for diagnosis and treatment. In particular, venous catheter placement and venous blood collection applications are the most painful and frightening processes for the child. Although these procedures are very short and not very painful, the presence of children in different settings and the lack of knowledge about the process increase the level of anxiety and pain, and children experience many emotional and physiological problems. Aim: This researh was conducted to determine the effect of music during the intravenous challenge on the pain, anxiety and physiological signs (pulse, oxygen saturation) of the 4-6 year old children who applied to hematology and oncology daily diagnostic and treatment center Materials and Methods: The sample of the study consisted of 52 children who were admitted to the hematology-oncology diagnosis and treatment unit of a state hospital in Istanbul, between 4 and 6 years of age and between March-June 2017 on Tuesdays -Firdays.Children(experimental group and control group)were randomly assigned according to the order of arrival. At the 5th minute, children in the experimental group were listened to pentatonic music while the procedure was being performed and the control group was routinely applied. In the collection of the data by theform that was prepared by the researcher , the FLACC Pain Scale was administered and the physiological parameters were measured before and during the procedure, and the Venham Anxiety Scale was administered before and after the procedure .Data were evaluated in SPSS 23.0 program using the descriptive statistical analyzes (number, percentage, mean, standard deviation, minimum-maximum value), Kolmogorov-Smirnov and Shapiro-Wilkinson, Man Whitney-U, Wilcoxon test and independent t test, 95% confidence and p <0.05 significance level. Results:there was no statistically significant difference between children and parents characteristics ( father's age and mother's education status out). It was seen that the control group had higher pulse rate rank averages and a statistically significant difference (p <0.05) when the experiment group had fallen with musical influence even with pulse value of invasive procedure. The mean value of the saturation values of the experimental group was found to be higher than that of the control group and it was found that the change was not statistically significant both within the group and between the groups (p> 0,05). It was seen that the average score of the pain score in the experimental group was lower during the procedure than the control group (p <0.05) and that the average score of the pain group of the control group was increased and statistically significant among them. According to the average scores of the anxiety points, the anxiety score of the experimental group dropped by the music according to the control group, the average level of anxiety score of the control group increased and the statistical difference between them was found to be statistically significant (p <0.05). Conclusion and Recommendations: The music (pentatonic music) is played before and during painful medical procedures and the children's attention is drawn to another direction; decreased pain and anxiety due to the procedure, and regulated the change in physiological parameters (reduced pulse rate)1. It is suggested to investigate the effectiveness of pentatonic music, to explain the effect in children's services and to spread the use especially in invasive interventions, when there are a large number of samples in different age groups. Keywords: Pain, anxiety, distraction strategy, pentatonic music
