Abstract (EN)

In this study, we aimed to evaluate the basic information of adult patients diagnosed with primary immune thrombocytopenia by the Hematology Department of Trakya University Faculty of Medicine between 2012 and 2022. Treatment-related parameters such as patients' age, gender, whether there was bleeding at presentation, concomitant treatments and chronic diseases they were using, blood counts at presentation, first-line treatment used, response to treatment, drugs used in second-line treatment, and whether splenectomy was performed were evaluated. It was observed that the 278 patients in our study were predominantly female, more frequently observed between the ages of 30-60, a similar incidence was observed over the years, but there were slightly more newly diagnosed cases in 2020-2021 compared to previous years. When bleeding was considered, it was observed that those with low platelet counts and in need of treatment experienced bleeding more frequently, but these bleedings were not life-threatening. It was observed that bleeding occurred more frequently below 10 x 109/L. It was observed that corticosteroids and especially methylprednisolone were used in the first step of treatment, as is the case all over the world. For second and subsequent stage treatments, splenectomy was required at a rate of 10,1% in all patients, thrombopoietin receptor agonists were used more frequently, and adequate responses were obtained. It was determined that 45% of patients required intravenous immunoglobulin treatment at least once in rescue and emergency treatments. It was observed that the number of cases in which thrombopoietin treatment completely discontinued was 8,3% and these cases were discontinued in 2021-2022. When considering accompanying medications and treatments, hypertension was observed at a frequency of 20,5% and diabetes at a frequency of 6,5%, and regarding thrombosis, which can be seen as a complication of both disease and treatment, arterial thrombosis was observed at a frequency of 2,9% and venous thrombosis at a frequency of 2,9%.It has been noticed that additional risk factors are more prominent in venous and arterial thromboembolic events. In this perspective, we think that it would be important to review coronary artery disease risk scores and venous thromboembolism risk factors in all patients individually and in repeated visits.
