Abstract (EN)
The standard treatment approach for locally advanced gastric tumors is surgery after neoadjuvant chemotherapy. In our study, we aimed to investigate the factors predicting treatment response and prognosis in locally advanced gastric cancer patients receiving neoadjuvant chemotherapy. Materials and Methods: A total of 106 patients with non metastatic gastric adenocarcinoma who were admitted to the Medical Oncology outpatient clinic of Yüzüncüyıl University Medical Faculty Hospital between January 01, 2010 and September 01, 2023 and who received neoadjuvant chemotherapy were included in this study. Demographic data, clinicopathologic features and various laboratory parameters of the patients were analyzed according to the information in the hospital records. Results: When the responses to neoadjuvant chemotherapy were evaluated according to the demographic and clinicopathologic characteristics of 106 patients in our study; clinical T stage was significantly lower (p<0.05), curative surgery rate was significantly higher (p<0.05), palliative surgery rate was significantly higher (p<0.05), surgical margin rate was significantly higher (p<0.05) in the group with pathologic response than in the group without pathologic response, neural and vascular invasion was significantly lower (p<0.05), intestinal lauren rate was significantly higher (p<0.05), good and moderate differentiation rate was significantly higher (p<0.05), involved lymph node was significantly lower (p<0.05), 05), higher TNM stage significantly lower (p<0.05), ECOG score significantly lower (p<0.05), neuropathy and stomatitis rate significantly lower (p<0.05), CA19.9 and PDW significantly higher (p<0.05), complete and partial radiological response rate significantly higher (p<0.05), 05), cap TRG 2017 score was significantly lower (p<0.05), the number of adjuvant treatment cycles was significantly lower (p<0.05), survival rate was significantly higher (p<0.05), and disease-free survival was significantly higher than the group without pathologic response. Conclusion: In conclusion, we found that patients who were given neoadjuvant treatment were less likely to respond to treatment due to high clinical T stage, poorly differentiated histology, high grade, high ECOG performance score, low serum CA 19-9, neuropathy and stomatitis as side effects. Expected survival was higher in patients who responded to neoadjuvant treatment.
