Abstract (EN)

ABSTRACT Introduction: Obesity is a multi-factorial condition, which is defined as accumulation of excess body fat and adipose tissue due to the imbalance between caloric intake and burn. The main problem associated with obesity is the risk factor of metabolic syndrome, which is accompanied by glucose homeostasis problems, central obesity, increased triglyceridesand hypertension. People are exposed to PCBs through inhalation, dermal contact, and ingestion of fish, vegetables and eggs contacted with municipal, hospital and industrial waste waters. The main resource of exposure is through ingestionand serum level of PCBs are affected mainly by environmental exposure through food. Because of their lipophilic character PCBs are deposited in adipose tissue, liver and pancreatic β-cells. As a result of this accumulation PCBs are associated with obesity and insulin resistance. The aim of this study is to show the relationship between obesity, insulin resistance and metabolic syndrome. Objective: To compare the blood level of obesogenic and endocrine disrupting chemical PCB in between children with exogenous obesity and control group and to Show the effect of PCB on the indices of insulin resistance and metabolic syndrome. The participants filled out a questionnaire to show the possible contact with PBC. Hypothesis: PCB exposure causes obesity, insulin resistance and metabolic syndrome.Serum PCB levels correlate with BMI SDS and adiponectin levels. Methods: This study included 50 exogenous obese patients without any chronic diseases between ages of 6-18 who admitted to Ege University Children Hospital Department of Pediatric Endocrinology and 50 control patients without any chronic diseases between ages of 6-18 who admitted to Ege University Children Hospital Department of Pediatric Emergency and Department and General Pediatrics. All patients who fulfilled this criterion were asked to participate in the study and the tests were performed at the first medical visit. Obese patients are routinely being tested for glucose tolerance. From all blood samples of the obese patients, basal and postprandial insulin levels, fasting and postprandial blood glucose, serum lipids (total cholesterol, triglycerides, LDL and HDL), serum adiponectin levels and serum PCB levels were studied. The same parameters in the fasting state were studied in the control group. Conclusions: BMI SDS, fasting blood glucose, HOMA-IR, total cholesterol, LDL and adiponectin levels were significantly different in between the two groups. PCB levels were negative in all of the patients and controls. Parameters of insulin resistance, lipids and adiponectin were not different between patients with and without metabolic syndrome. ROC analysis has proved that adiponectin can predict the obesity. Keywords: obesity, PCB, adiponectin, insulin resistance, metabolic syndrome
