Evaluation of the relationship between homa-ir and triglyceride/glucose index and plasma atherogenic index in patients admitted to the obesity clinic

Abstract (EN)

Objective: In this study, it was aimed to compare the relationship between HOMA-IR measurement and Triglyceride Glucose index (TyG) and Plasma Atherogenic Index (PAI) in patients admitted to the obesity outpatient clinic. Method: 205 patients who applied to Düzce University Family Medicine Obesity Polyclinic between 01.01.2022 and 31.12.2022 were included in this study. HOMA-IR, TyG and PAI values were calculated using the laboratory data of the patients and their relations with each other were evaluated. Results: 205 patients were involved in our study, and 78.5% of them were female. It was determined that both women and men were mostly in the Stage 1 obese group. It was observed that there was a significant increase in weight, FPG, LDL, insulin, HOMA-IR and TyG values as BMI increased. According to the HOMA-IR value, the weight, BMI, FPG, Triglyceride, Insulin, TyG, and PAI values measured in the group with insulin resistance were found to be significantly higher than the other group (p<0.001, p<0.001, p<0.001, p=0.044, p<0.001, p=0.004, p=0.003 respectively). Age, FPG, LDL, T. Cholesterol, Triglyceride, Insulin, HOMA-IR, PAI values measured in the insulin resistance group according to the TyG index were significantly higher than the values measured in the other group (p=0.006, p=0.003, p<0.001, p<0.001, p<0.001, p<0.001, p<0.001, p<0.001 respectively). LDL, T. Cholesterol, Triglyceride, HOMA-IR values measured in the high-risk group according to PAI were found to be significantly higher than the values measured in the other groups ((p<0.001, p=0.008), (p<0.001, p=0.001), (p<0.001, p<0.001), (p=0.013, p=0.038) respectively). Conclusion: In our study, we found that HOMA-IR, which is a widely used indicator of insulin resistance in accordance with the literature, is in a correlation with the new generation TyG and PAI indices. The situation where insulin resistance cannot be determined due to the inability to measure the insulin level in most units where primary laboratory services are provided can be eliminated by using the TyG index, and the cardiometabolic risks of the patients can be easily evaluated with PAI.
