Evaluation of patients developed disseminated intravascular coagulopathy by obstetrical causes

Abstract (EN)
In our study, we aimed the guiding to physician antepartum evaluation the patients who develop DIC with obstetric reasons, prediction and prevention of complications, and the presentation of information to reduce maternal morbidity and mortality. Patients who delivered in our clinic and/or the external center or post-natal transferred to the clinic were included between July 2006 and June 2013. Patients diagnosed with obstetric DIC reasons were included in the study and DIC score was made according to the International Society on Thrombosis and Haemostasis (ISTH) criteria. Patients were divided into four groups according to the etiology. These were classified group-1 (hypertensive pregnancies), group-2 (post-partum bleeding ones), group-3 (placental abruption patients) and group-4 (among others). External center giving birth and follow-up of patients requiring surgical interventions in the postpartum period, transferred times to our clinic and after evaluating the patients in our clinic to be surgically again and surgical interventions were noted. Sequelae and complications occurring after discharge, causes of mortality in patients with ex were recorded. 52 patients diagnosed with obstetric DIC reasons included the study. Twelve our patient is ex due to DIC and we detected mortality rate 23.1% (12/52).The incidence of DIC due to obstetric reasons were found to be 0.314%. During the recourse, calculated DIC scores were found to be appropriate (? 5 points) in all ex patients with overt DIC criteria. We detected ex patients with an average value (5.75 ± 2.34) and these values were observed higher than the other patients (4.97 ± 2.00) but this was not statistically significant (p = 0.303). The prognosis of DIC cases have limited a small number of studies. According to our study, increased INR values (> 2.0) and at the follow-up, increasing value of LDH suggest the prediction of mortality prediction but we believe that to be larger series of studies.
