Abstract (EN)
Peripheral artery disease is the chronic and progressive deterioration of lower extremity arterial blood circulation due to systemic atherosclerosis. This disease is currently a public health problem because it is known to be associated with increased mortality and morbidity. Revascularization surgical techniques in the treatment of patients with this disease have gained different perspectives with the developments in angiographic imaging methods that have gained momentum in recent years. The superiority of these gains over each other is still a matter of debate. Our study was prospectively conducted between 15.09.2024 and 15.03.2025. 52 patients who applied to the Trakya University Cardiovascular Surgery Department outpatient clinic with complaints of inability to walk and who needed revascularization and met the inclusion criteria were included in the vascular surgery program for the application of open surgery and endovascular procedures suitable for them. Blood samples were taken from the patients before the planned procedure, at the 1st hour after the procedure, and on the 1st day after the procedure, and ischaemia modified albumin was examined. The physical examination findings, detailed anamnesis, demographic characteristics, radiological imaging, and ankle brakial index values of the patients were examined. Our aim in this study was to evaluate the results of patients with peripheral artery disease after different surgical options with ischaemia modified albumin. In this way, it was aimed to have an idea about the superiority of different surgical options and to reveal the functionality of ischaemia modified albumin in peripheral artery disease follow-up. 15 (28.8%) of the patients were deemed suitable for open surgery, and 32 (71.2%) were deemed suitable for endovascular procedure. 61.5% of the patients had hypertension, 59.6% had diabetes mellitus, 46.2% had coronary artery disease, and 15.4% had chronic renal failure. The smoking rate was 78.8%. These rates were determined to be above the literature, and peripheral artery disease requires a multidisciplinary approach due to accompanying diseases. When revascularization results are evaluated with ischaemia modified albumin, we believe that open surgery cannot be used in the early period due to the vascular clamping required by the procedure, but we can have an idea about the ischemia burden on the basal ischaemia modified albumin value and it can be evaluated quantitatively in outpatient clinic controls. We see an early decrease in ischaemia modified albumin in endovascular procedures, which both reveals the success of the procedure and reveals the option of quantitative follow-up for early complications. The usability of the parameters that can be used in measuring the success of revascularization surgery in peripheral artery disease, which should be evaluated with a multidisciplinary approach and should be considered as a public health problem, should be evaluated, and additional detailed studies with a high number of patients are needed in this direction.
