Abstract (EN)
Effects of Preoperative Oral Gabapentin in Ultrasound-Guided Infraclavicular Brachial Plexus Block Aim: For anesthesiologists, it is very important to patients with comfort and safety during surgery. Preoperative administration of gabapentin is known to induce analgesic activity and is known to reduce the consumption of analgesics. In this study, two different doses of preoperatively applied gabapentin in the upper extremity surgery are compared for their postoperative analgesic effects. Materials and Methods: With the approval of the Local Ethics Committee approval by Çukurova University Faculty of Medicine, urology and cardiovascular surgery patients who require opening of arteriovenous fistula, Plastic and Reconstructive Surgery patients who require flap debridement and orthopedic surgery patients who require upper extremity operations are consider for the study covering a total of 60 patients for whom infraclavicular brachial plexus block was performed. Group 1: (n = 20): 30-45 minutes after administration of oral placebo tablets, 40 ml of 0.5% bupivacaine and 0.5 ug / kg fentanyl used Group 2: (n = 20): 30-45 minutes after administration of oral 600 mg Gabapentin, then 40 ml of bupivacaine and 0.5 ug/kg fentanyl used Group 3: (n = 20): 30-45 minutes after administration of oral 1200 mg Gabapentin, then 40 ml of 0.5% bupivacaine and 0.5 ug / kg fentanyl used Intraoperative and postoperative hemodynamic parameters 1, 15, 30, 60 minutes, 2, 4, 6, 12, 18 hours, hemodynamic parameters, pain levels (VAS), analgesic requirements, and side effects of Ramsey sedation levels were recorded. Postoperative follow-up for 2 hours, afterwards patients are admitted to Orthopaedics and Traumatology service where follow-up was continued until 24th hours. Findings: Demographic data for groups were similar. For the first 30 minutes and for between 4-24 hours, there were no significant differences between the groups in terms of the pain scores. In other timer periods, VAS values were lower in the placebo group. Postoperative tramadol consumption and sedation scores did not differ between the groups. Result: It is concluded that preoperative oral administration of 600 mg and 1200 mg gabapentin in infraclavicular brachial plexus block applied in the upper extremity surgery found to cause no hemodynamic changes, no effect on the level of sedation, postoperative pain scores and tramadol consumption.
