Comparison of cardiovascular and respiratory parameters of ECT applied with and without using rocuronium with propofol

Abstract (EN)
Objectives: We aimed to compare patients who were used propofol during anesthesia induction with patients who were used rocuronium and propofol in terms of cardiac parameters (HR, MAP, ECG), respiratory parameters (SpO2, PI, PVI) and side effects (myalgia, nausea-vomiting, tachycardia-bradycardia, hypertension-hypotension, arrhythmia allergy) in patients who underwent ECT. Materials and Methods: In our study, 84 patients with ASA I-II schizophrenia, major depression and bipolar disease were included. Heart rate (HR, beats / minute), PVI, PI, SpO2, MAP (mmHg) and ECG of the patients were recorded both before induction and at the end of convulsion. P, Pd, QRS, QT, T values were checked in the ECGs of the patients. In our study, patients in the R group (n = 42) were administered with 2% lidocaine IV 0.5 mg / kg, propofol IV 1-2 mg / kg and rocuronium IV 0.2 mg / kg for induction. After convulsion, decurarization with sugammadex 2 mg / kg was applied. Patients in the P group (n = 42) were given 2% lidocaine IV 0.5 mg / kg and propofol IV 1-2 mg / kg only for induction. After ECT, patients were followed up in the recovery unit, and side effects (myalgia, nausea-vomiting, tachycardia-bradycardia, hypertension-hypotension, arrhythmia, allergy) were observed and recorded. Results: There was no statistically significant difference between group P and group R in terms of demographic data (p>0,05). QT interval after convulsion was statistically longer in female patients compared to the male patients (p = 0.034). Pd values before induction were statistically lower than Pd values after ECT in both group P and group R (p=0,001). In both patients in the P group and the R group, the post-ECT values for HR were statistically higher than the pre-induction values (p=0,001). Preinduction MAP values were statistically lower than post-ECT MAP values in both group P and group R (p=0,001). The recovery time was statistically longer in group R than group P (p=0,001). In terms of side effects, myalgia was seen in 14 (33%) patients in group P and in 4 (9%) patients in group R, and it was statistically significantly higher in group P (p=0,001). SpO2 value before induction was statistically higher than SpO2 value after ECT in both group P and group R (p=0,001). Conclusion: In ECT application of propofol anesthesia without the use of muscle relaxants, rapid recovery was provided, no additional side effects were seen except for postoperative temporary myalgia, thus it was concluded that it provided sufficient and safe anesthesia.
