Abstract (EN)
Recent years, specialists of Chest Diseases interest and the need for intensive care has been increasing. Because of the high mortality in intensive care units care and follow-up are carried out more frequently. Determination of mortality prediction scoring systems which enables early detection of patients with high mortality and routine tests of the stimulus about mortality high risk patient is important to detect. The laboratory data and vital signs of 74 patients that measured within 24 hours of admission were prospectively recorded on specially crafted files of patients in Dicle University Medical Faculty Hospital of Respiratory İntensive Care Unit. Patients who stayed less than 24 hours in our intensive care unit, younger than 18 years, burn patients and patients who had undergone coronary surgery were excluded from study. APACHE II, APACHE III, APACHE IV, SAPS II, MPM II and SOFA scorings were calculated on computers with these obtained data. A statistical software package (SPSS 15.0; SPSS Inc, Chicago, IL) was used for statistical analysis. Patients were classified as death and the alive. Student's t test and chi-square test were used in comparison of admission initial data of the death and alive patients. Parametric data contained in with Student t test significant difference between alive and death group were subjected to ROC analysis. The area under the ROC curve (AUC) was calculated. The significantly different categorical and numerical data was analyzed with conditinal backward binary logistic regression method, and their contribution of mortality was investigated. Intensive care scoring systems and univariate and multivariate analyzes were performed using the APACHE II, III, IV, SAPS II, MPM and SOFA intensive care scoring systems. In statistical analysis with 95% confidence intervals and p <0.05 was considered significant. Different mortality scoring systems are available for intensive care units. In this study, we compared APACHE II, APACHE III, APACHE IV, SAPS II, MPM II and SOFA scoring systems in our respiratory intensive care unit. All scoring systems have been found to be statistically significant for predicting mortality. According to ROC analysis mortality predicting effectivity were found as APACHE IV > APACHE III > SAPS II > SOFA> MPM II >APACHE II. The difference between routine tests as hemoglobin, hematocrit, serum albumin, serum total protein, BUN levels were statistically significant in the death and alive group.
