Abstract (EN)

Introduction and Aim: Brugada Syndrome, which is characterized by typical EKG findings and may cause sudden cardiac death, is associated with SCN5A mutations, approximately 1/3 of the cases. SCN5A encodes the Nav1.5 protein, the alpha subunit of the cardiac sodium channel. SCN5A is expressed to show close to 100% homology to the heart in intestinal smooth muscle and interstitial Cajal cells from non-cardiac tissues. There is an increase in the frequency of SCN5A mutations in patients with Irritable Bowel Syndrome (approximately 2% of cases), and these mutations are actually related to deterioration in voltage characteristics and mechanosensory characteristics in intestinal tissues. The aim of our study is to investigate whether the symptoms of functional intestinal symptoms increased in patients with Brugada syndrome which is a clinical diagnosis beyond the molecular diagnosis in SCN5A function loss mutations or Type 1 Brugada EKG pattern. Materials and Methods: Type 1 Brugada EKG pattern which was previously drug or spontaneously and 148 cases not meeting the exclusion criteria Eighty-six patients with cardiac arrhythmias associated with Brugada EKG and 88 patients with a negative predictive of ajmaline induction were enrolled in the study. In each group, who were our basic demographic data variables of gender and age group were evenly distributed in such a way could be compared easily. Refers to the criteria for non-compliance with the exclusion criteria in the groups and to the diagnostic criteria for ROME III functional bowel diseases and a questionnaire that led to the diagnosis validated for the Turkish society was filled out by translating into Turkish. The questionnaires were evaluated by the researcher who was blind. We evaluated the statistical significance of IBS, functional constipation, functional diarrhea and functional swelling in the cardiac diagnostic groups. Symptoms: The differences between the percentages in the cross-table analysis of functional bowel diseases were statistically significant and seemed to be due to the increased IBS frequency in the foreground Type 1 Brugada EKG pattern group. IBS frequency in patients with Type 1 pattern is 25% (37/148), in individuals with arrhythmia specifications and with a negative appearance of ajmaline test 11.8% (8/68, for healthy controls was detected as 5.3% (3/57) . increase was almost suitable to the geometric pattern and meaningful as statistically (P=0.001). When the arrhythmic groups were evaluated in themselves (25.0% versus 11.8%), along with the ajmaline positivity did not provide an additional increase, the increase (P = 0.07) was close to significance and was thought to be due to the low number of arrhythmic-ajmaline negative cases. In the Brugada group, there was no difference in Schanghai scores in patients with functional bowel disease and IBS. Probably functional complaints are not associated with the severity of the Brugada phenotype, but each mutation is associated with the block ability and defect of the SCN5A channel only because it has a different pathogen character. Conclusion: Functional bowel complaints are significantly higher in people with type 1 Brugada EKG pattern, which is a clinical sign of having a defective sodium channel. Beyond a cardiac diagnosis, Brugada Syndrome may be part of a multisystem genetic channelopathy. In order to prevent sudden cardiac arrest, drugs for gastrointestinal complaints should be evaluated in terms of sodium channel blockage characteristics and Brugada syndrome.
