Retrospective evaluation of the effect of dapagliflozin therapy on inflammatory markers in Type 2 diabetes mellitus patients

Diabetes, a global public health problem, is a chronic metabolic disease characterized by hyperglycemia. SGLT2 inhibitors are relatively new oral antidiabetic drugs that act by reducing glucose reabsorption in the renal proximal tubule and increasing urinary glucose excretion. Low-grade chronic inflammation is known to play an important role in the pathogenesis of type 2 diabetes and related complications. Recently, there are studies showing that hemogram parameters such as RDW, MPV, PDW and ratios such as NLR, PLR, MLR, MHR can be used as a systemic inflammatory marker with the advantages of being cost-effective and easy to calculate. SGLT2 inhibitors are thought to inhibit the release of proinflammatory mediators independently of their antihyperglycemic effects. In our study, we aimed to record the changes in inflammatory markers after dapagliflozin in type 2 diabetes patients. This retrospective study included 114 patients with type 2 diabetes mellitus. Laboratory values of the patients were recorded before dapagliflozin and after at least 12 weeks of drug use. The median age in the study was 59.5 (31-82) years. 58.8% (n=67) of the patients were female and 41.2% (n=47) were male. After dapagliflozin, a significant decrease in HbA1c, fasting plasma glucose, microalbuminuria, ALT values; and a significant increase in Hgb, Hct values were found in line with the literature. While no significant change was observed in NLR, PLO, MLO, and MHO values, a significant increase in RDW values and a significant decrease in MPV, PDW values were detected. There are many studies in the literature examining the relationship between diabetes and its complications and new inflammatory markers. Considering the relationship between diabetes and inflammation and the effects of dapagliflozin on inflammation, prospective studies with larger series are needed.
