Abstract (EN)
Pregnancy is a period in which maternal and fetal health are threatened by various infections and many physiological changes take place. Infection in pregnancy and its causing congenital anomalies are one of the most important causes of mortality and perinatal morbidity, especially in developing countries. It is known that TORCH group infections during pregnancy which cause aborts or stillbirths are caused by Rubella, Cytomegalovırus, Toxoplasma gondii and other factors. Early detection of these infections during pregnancy is quite important for fetal health. The aim of this study is to identify seroprevalence of Rubella, Cytomegalovirus and Toxoplasma gondii in pregnancies, determination of stillbirth and preterm delivery rates and comparison of IgG avidity with avidity in pregnant patients with positive IgG antibodies. The study was conducted on 300 pregnant patients between 18 and 48 years old who applied to the policlinic of Obstetrics and Gynecology, Dursun Odabaş Medical Center, Van Yüzüncü Yıl University between September 2015 and September 2017 in Van province. The gestational age was between 18-48 and the average age was 29 ± 6.9. Gestational week was between 11-32 weeks and gestational week was 21.5 ± 3.4. The mean number of total pregnancies (gravida) was 3.2 ± 1.6. When the pregnancies were examined according to the residence places, it was seen that 196 (65,3%) reside in the city while 104 (34,6%) reside in the rural area. It was observed that 126 (42%) of women graduated from primary school and 52 of women (17,3%) graduated from high school and post-secondary schools, while 122 (40.6%) of them were not literate. The ELISA method was used to investigate Cytomegalovirus, Rubella and T. gondii as well as antibodies of IgG and IgM in 300 blood samples from pregnant patients included in the study. Rubella IgG 96% and Rubella IgM 4%, CMV IgG 99%, CMV IgM 1.3%, T. gondii IgG 56.3% and IgM 7.6% seropositivity were detected in pregnant blood samples. The ratios of stillbirth and preterm delivery are respectively determined as Rubella IgM 2 (18.1%) / 4 (17.3%), Rubella IgG 11 (100%) / 22 (95.6%) and CMV IgM 1 (9%) / 2 (8,3%), CMV IgG 11 (100%) / 23 (100%), T. gondii IgM 1 (9%) / 3 (13%), T. gondii IgG 4 (34.7%). The avidity values were determined as 3/0/2 in Rubella, 0/0/3 in Cytomegalovirus and 8/1/12 in T. gondii in low / gray zone / high values respectively. As a result, Toxoplasma gondii, Cytomegalovirus and Rubella, which may cause aborts or stillbirths, are among the important health problems in pregnant patients in our region. It has been concluded that serologic methods may be a method of guiding medicine in diagnosing these infections and clarifying suspicious cases by performing IgG avidity test. In addition, it was concluded that educating the community about protection, transmission routes and controlling of these diseases is very important for community health.
