Abstract (EN)
Introduction and Aim: Kidney failure is a common problem in patients in reanimation units. Renal replacement therapies are needed for the treatment of this pathological condition, which is characterized by decreased kidney function. In this study, we aimed to retrospectively evaluate the effect of Crp/Albümin ratio on mortality in patients who underwent continuous renal replacement therapy (CRRT) in our intensive care unit. Material and Method: Digital archive data of Harran University Anesthesiology and Reanimation Clinic Intensive Care Units, doctor anamnesis, laboratory results and nurse observation forms of the patients were examined. The demographic characteristics of the patients, the reasons for their admission to the intensive care unit, whether they took inotropes during CRRT, if they were taking inotropes, which agent they took, and whether they needed mechanical ventilation during CRRT were examined. Discharge and death status of the patients were evaluated. The total length of stay of the patients in the intensive care unit and hospital was recorded. It was noted how many sessions of CRRT the patients received, how many ml of UF was applied during CRRT, and how many hours of CRRT they received. The CRRT modality (SVVHD, SVVH, SVVHDF, YSUF) applied to the patients was examined. Preferred solutions for CRRT were examined. Concomitant diseases of the patients were examined. Acute Physiology and Chronic Health Assessment II (APACHE II), GCS, Sequential Organ Failure Assessment (SOFA), acute kidney injury scoring systems (RIFLE and KDIGO), glucose, urea, creatinine, albümin, K, Crp/Albümin ratio, Ph, WBC, Hgb, PLT, RDW and MPV were measured. XII Results: A total of 107 patients were included in our retrospective study between 2017 and 2022, and 43% of the patients were female and 57% were male. When the comorbidities of the patients in our study are examined, it is seen that hypertension (HT) is the first frequency with 42.1%, and diabetes mellitus (DM) is the second frequency with 31.8%. In addition, in our study, the mean APACHE II score of the cases was found to be 22.00±9.00 at ICU admission. It was found to be 31.00±6.00 before CRRT. In our study, the mean SOFA score of the cases was found to be 6.00±3.00 at hospital admission and 10.30±2.10 before CRRT. In our study, no relationship could be established between the APACHE II score in ICU admission and mortality. In our study, 97.2% of our patients were connected to a mechanical ventilator during CRRT. When we compared the laboratory parameters measured before and after CRRT application of our patients, we found that urea, creatinine, CRP and potassium values improved significantly. We found that 82.2% of our patients needed inotropic support during CRRT. In our study, a statistically significant difference was found between mortality rates in cases according to CRP/Albumin levels. Conclusion: 101 of our 107 cases in our study resulted in mortality. In our study, the CRP/Albümin values of our patients before and after CRRT did not differ statistically according to mortality. In our study, the Crp/Albümin values of our patients who died in the intensive care unit admission and discharge follow-ups were found to be statistically significantly higher than those who were discharged.
