Özet (EN)

Myocardial Perfussion Scintigraphy and Effort ECG Testing Activity On The Patients Which Have Atypical Chest Pain Diagnosis In Emergency DepartmentChest pain patients compose one of the most important parts of emergency visits. Despite the existence of highly sensitive and specific cardiac markers, 2-5% of patients with acute myocardial infarction discharged from emergency departments inadvertently. In our study we have investigated patients with atypical chest pain diagnosis, who were admitted to emergency department and discharged ? after excluding the patients with life threatening situations. On the follow-up of the patients, our aim in the study was to show the efficiency of myocardial perfussion scintigraphy and Effort ECG Testing to determine possible coronary artery diseases of these patients. In this anterograde study with the consent of the Ethics Commission, 50 patients (aged +18) which have discharged from Cukurova University Medical Faculty Emergency Department with atypical chest pain diagnosis were reviewed.Of the 50 patients included in the study; 46% (n=24) woman, 54% (n=26) man and average age was 43±11,85. When the description of the chest pain was asked to these patients 30% of the patients replied as stinging-like, 50% as pressure-like, 20% as burning-like. Most frequent clinical symptom which accompanied chest pain was sweating (56%). Most frequent medical history finding was tobacco smoking (48%). During the follow-up period of the patients there were no change detected in ECG or biochemical indicators. The Effort ECG Testing results of the patients were reported as; 88% (n=44) negative, 4% (n=2) pathological, 6% (n=3) suboptimal investigation due to low Effort Ecg Testing results. Coronary angiography was performed on the 4% (n=2) of the patients who had MPS Spect result as pathologic. One of the patients had normal result from coronary angiography other one had newly diagnosed coronary disease as the result. We conclude that; Effort ECG Testing and Myocardial Perfussion Scintigraphy test would be effective if it was performed in early stages to lower the morbidity risk of the patients, who are older 65 years, tobacco smoking and have risk factors as systemic hypertension and to detect the overlooked, undesired cardiac events. Keywords: Atypical Chest Pain, Exercise Stress Testing, Myocardial Perfüssion Scintigraphy, Emergency Department
