Abstract (EN)

Objective: We aimed to investigate the prognostic utility of thiol disulfide levels, a new oxidative stress parameter, in critically ill patients applying emerceny room and requiring intensive care. Material and methods: In this study, it is evalvated prospectively that critically ill patients need intensive care who applied to Yıldırım Beyazıt Üniversity Atatürk Training and Research Hospital Emergency Service casuality department between the dates November 2017 to April 2018. The study has inclueded 79 patient as 'patient group' and 71 healty volunteer 'kontrol group' of similar age group with no complaint These ill patients were analysed according to age, gender, presenting semptoms, states of consciousness, acute physiological condition, values of native thiol, toplam thiol, disülfid, ischemia modified albumin and ferroxidase. In addition, ortalama arterial pressure, glaskow coma score (GCS) and APACHE 2 scores were calculated for all ill patients. It was determineted the status after 28 days. Results: Patients and kontrol groups were divided into groups according to their gender and examined statistically. 39.2% (n: 31) of the patients were female and 60.8% (n: 48) were male. 38% (n: 27) of the kontrol group were female and 62% (n: 44) were male.There was no statistically significant difference between two groups in gender distribution (chi-square test, p: 0.879). Native thiol and toplam thiol values of EREL parameters were significant when compared to the 28-day mortality. 28 days later, ortanca value of native thiol in the all patient group was detected 277 and 314 were found in living patients and the ortanca value of thiol 228 was found in exitus and it was statistically significant ( p<0,001 ). Also 28 days later, ortanca value of toplam thiol in the all patient group was detected 322,5 and 363,3 were found in living patients and the ortanca value of t hiol 266 was found in exitus and it was statistically significant ( p<0,001 ). APACHE 2 mortality rates were significantly associated with 1 month mortality and the overall mortality ortanca was 22% in the patient sample, ortanca 16% in those living after 1 month, and ortanca 42% in exitus ( p<0,001 ). NT and TT levels from the EREL parameters, which are oxidative stress indicator were negatively correlated with APACHE 2 score and APACHE 2 mortality rates Conclusion:NTand TT levels of oxidative stress indicator EREL parameters were statistically significant correlations with 28 day mortality and negative correlations were found with APACHE 2 scoring and APACHE 2 mortality rates and it is suggests that there may be a significant parameter to determine the patient prognosis.
