Effects of therapeutic approaches on intubation response, agitation behavior, and extubati̇on ti̇me in patients undergoing open heart surgery

This research was planned in a post-test control group research design in order to determine the effectiveness of the training given before and after the surgery according to the therapeutic communication stages. The sample of the study consisted of 30 patients, 15 of whom were experimental and 15 of whom were control, from open heart surgery patients hospitalized in a foundation university hospital in Istanbul. The data were collected between 10.10.2022 and 07.01.2023 with the sociodemographic information form prepared by the researcher, RASS (Richmond Agitation Sedation Scale), Intubation reactions observation form. Numerical anxiety scale (control variable) was applied to both groups preoperatively. A training program based on the therapeutic communication process structured in accordance with the process was applied to the patients in the experimental group (n=15) before and after the surgery. The control group (n=15) received standard care. Postoperative RASS and Intubation responses observation form was applied to both groups. Measurements were made when patients first woke up after the sedation procedure in the intensive care unit. In the analysis of the data, frequency and percentage analyzes were used to determine the descriptive characteristics of the employees participating in the research, and average and standard deviation statistics were used in the analysis of the scale. Kurtosis (Kurtosis) and Skewness (Skewness) values were examined to determine whether the research variables showed a normal distribution. Differences between the ratios of categorical variables in independent groups were analyzed with Chi-square and Fisher's exact tests. The differentiation of continuous variables according to groups was analyzed by independent groups t-test. The intubation responses of the patients did not differ significantly in the likert measurement compared to the experimental and control groups. However, when the intubation responses were evaluated as seen/not seen statistically, the score of the control group compared to the experimental group in "wrinkled facial expression" (X2=5,000; p=0.030<0.05) and "picking sheets with hands" (X2=6.652; p=0.013<0.05) responses was found to be significantly higher. There was no significant difference between the experimental and control groups in terms of RASS scores and time to leave intubation (p>0.05).
