IVABSTRACTTHE EFFECTS OF PREOPERATİON PERFORATİON AND SYPURATİON OFPULMONARY HYDATİD CYSTS ON SURGİCAL TREATMENT ANDMORBİDİTYDr. Mahmut Gulgosteren, Residency thesis, Depertmant of Thoracic SurgerySupervisor: Associate Professor.Dr. Bulent Tunçozgur.December 2006. 57 pagesDemolishing of the germinative membrane integrity in hydatid cysts iscalled as perforation. We hypothesized that there may be other differences(duration of hospital stay, surgery, morbidity, mortality) besides symptomatologicparameters between perforated and non-perforated groups. We aimed to clarifywhich parameters may be different between two groups by using appropriatestatistical tests in the determined parameters. We aimed to emphasize thatdifferences between two groups may affect surgical approach and post-operativemorbidity. We investigated the records of 169 patients who had been treated forhydatid cyst in our clinic between January 1995 and December 2005retrospectively. Cases were classified as Group 1 and Group 2 (non-perforatedand perforated hydatid cysts). Hospital admission durations, settlement areas, sexualities, symptoms and hospitalisation durations of the patients werecompared with each group. Statistical analysis was performed by using chi-square, and t-test. When the results are evaluated, cough, sputum production, hydoptisis (p<0,001), pain, fever, hemoptysis (p<0,05), and post-operativecomplications (p=0,002) had been seen more frequently in Group 2. All of thesedifferences are statistically significant. In conclusion, , major differences are observed between non-perforatedand perforated hydatid cysts. Hospitalisation duration and genesis of post-operative complications increase morbidity seriously in the perforated group. Forthis reason, pulmonary hydatid disease, a serious problem of public health in ourcountry, should be treated surgically as soon as diagnosed.Key words: Pulmonary hydatid disease, Perforation, Surgery, Morbidity.
