Abstract (EN)

Investigation and Comparison of Parameters Affecting Survival in Liver Transplant Patients at Akdeniz University Objective: Liver transplantation is one of the most effective treatment methods with a significant impact on reducing mortality. This study aimed to investigate the relationship between pre-transplant evaluation parameters and post-transplant survival, and to assess the predictive value of these parameters in estimating survival outcomes. Materials and Methods: This retrospective study included 439 patients over the age of 18 who underwent liver transplantation at the Akdeniz University Hospital Organ Transplantation Center and were followed up between January 2014 and August 2024. Data were collected based on the patients' most recent pre-transplant laboratory results and physical examination findings. Scoring systems such as Child-Pugh Score (CPS), Model for End-Stage Liver Disease (MELD), MELD-Na, MELD 3.0, ALBI, PALBI, and FIB-4 were calculated. The association between these scores and survival outcomes was analyzed by comparing low and high score groups, as well as deceased and surviving patients. Results: Of the patients included in the study, 70.4% (n=309) were male and 29.6% (n=130) were female. Among the donors, 77.4% were living and 22.6% were deceased. Patients who received grafts from older donors and those who were of older age themselves had significantly shorter survival times (p<0.011 and p<0.003, respectively). Patients with decreased ALP and GGT levels and higher albumin levels were found to have significantly better survival outcomes (p<0.035, p<0.004, and p<0.044, respectively). While no statistically significant difference was observed in survival duration between deceased and surviving patients for CPS, MELD, MELD-Na, MELD 3.0, ALBI, and FIB-4 scores, a statistically significant difference in both median and total survival days was found for the PALBI score. Conclusion: Although post-transplant complications play a major role in survival outcomes, this study demonstrated that scoring systems based on pre-transplant laboratory and clinical findings may have a predictive effect on survival duration. Particularly, the PALBI score showed significant predictive value and may serve as a useful tool in identifying patients who would benefit most from liver transplantation.
