Can the negative exploration ratio in patients who have been operated asacute appendicitis be decreased by use of Procalcitonin, C-RP and Neopterin ornot ?Acute appendicitis is the most frequently surgical pathology in the patientswho come to the emergency services because of acute abdominal pain. Nowadays,negative appendectomy (NA) and high rate of perforation are still being a problem,because of insufficient symptoms of acute appendicitis that confused with many otherdiseases.The tests for diagnosis and the images which are invazive and not practical arestill being in a developing period.In this study procalcitonin (PCT) that has used as a differential diagnosticmarker of inflamation and has inducted with sistemic reaction of organism in theinfection and C-RP which reflects acute phase stimulus prevalences and activity verywell and NPT which is secreted from macrophages and reflects the activations ofmacrophage were studied.100 patients whose ages were older than 16 and who were not pregnant cameto the emergency service with right lower quadrant abdomen pain longer than 12 hourswere hospitalized as acute appendicitis presumptive diagnosis. Same 100 patients,whose mean age were 34, were composed of 44 women, and 56 men. And their serumexamples were optained for PCT, C-RP and NPT analysis and the results wereevaluated with the SPSS 12.0 tests.All patients were separeted into 5 groups. Group I had hospitalized and whiletheir clinical symptoms were dissappered during hospitalization period weredischarged. Group II were operated with patological verification as A. vermiformis.Group III has diagnosed as acute appendicitis. Group IV has diagnosed as acuteflegmonous appendicitis and localized peritonitis. Group V has diagnosed as acuteappendicitis with plastrone and localized abcess.Group lll patients who were diagnosed as acute appendicitis, serum PCT, C-RPand NPT levels, Group IV patients PCT, C-RP levels and Group V patients PCT levelswere significantly higher than other groups levels.The serum PCT, C-RP and NPT levels can be practical, objective, noninvazivediagnostic tests with the patients that have abdominal pain history more then 12 hours.We thought that serum PCT, C-RP and NPT levels can be impressed diagnosticmethod when expecially laboratory and radiographic signs are not enough.Key words : Acute appendicitis, diagnosis, procalcitonin, C-reactive protein,neopterin.
