Operative delivery due to acute fetal distressevaluation of corelation of fetal monitorization and fetal umblycal artery blood gas and apgar score

In this study, cardiotocogram tracing was evaluated during delivery, and the correlation of neonatal umbilical cord blood gases and APGAR scores with traces during labor was evaluated in patients who were delivered by normal cardiotocogram trace and who underwent operative delivery due to acute fetal distress and it was aimed to determine how reliable cardiotocogram traces are used to make acute fetal distress decision. Within the scope of the prospective study, the data of the patients who gave birth in the Obstetrics and Gynecology Department of Bursa Uludağ University Medical Faculty Hospital between January 1, 2017 and October 31, 2018 were recorded using patient files and hospital automation system. 596 out of 1100 registered patients were included in the study because they met the study criteria. Demographic data, obstetric information, delivery methods and birth weight of the newborn taken in the delivery room, pH value of umbilical cord, APGAR score of 1 minute, APGAR score of 5 minutes, APGAR score of 10 minutes were recorded from hospitalization to delivery time of the patients included in the study. Statistical analysis of the data was performed by SPSS 23.0 for Windows computer program, t-test, ANOVA, chi-square and logistic regression analyzes. According to the findings of the study, a 1-unit increase in parity reduces the risk of acute fetal distress (AFD) by 61%. A 1-week increase in delivery week increases the risk of AFD by 1.4 times. A 1 gram increase in birth weight reduces the risk of AFD by 1%. The presence of chronic hypertension increases the risk of AFD by 3.23 times. Presence of pregestational diabetes mellitus increases the risk of fetal umbilical artery pH to be below 7.20 by 11.71 times. Presence of preeclampsia increases the risk of fetal umbilical artery pH being below to be 7.20 by 5.14 times.
